PERMIT #: M-1980-244 <br />INSPECTOR’S INITIALS: TC1 <br />INSPECTION DATE: November 10, 2015 <br /> <br /> <br />Page 2 of 8 <br /> <br />OBSERVATIONS <br /> <br />The Division conducted a monitoring inspection of the site on November 10, 2015. Mr. Chris Hanks represented <br />the Operator during various parts of the inspection. Tim Cazier represented the Division. The following facilities <br />were inspected during this site visit: <br /> Squaw Gulch Underdrain Ponds – installed liner and sump; <br /> Arequa Gulch Valley Leach Facility (AGVLF) - agglomerated tails placement, tension cracks; <br /> Squaw Gulch Valley Leach Facility (SGVLF) – construction status; <br /> Appurtenant Environmental Protection Facilities (EPFs): <br />o AGVLF water level check. <br /> <br />Squaw Gulch Underdrain Pond Inspection: <br />The recently installed underdrain liner (see Photo 1) was observed. Mr. Hanks pointed out mine personnel had <br />recently safely rescued a deer in one of the underdrain ponds that was unable to climb out on the HDPE -lined <br />3H:1V slope. He stated the mine had developed a means to provide egress for wildlife by fusion welding a <br />geoweb product in corners of the ponds and filling the geoweb with crushed stone. <br /> <br />Water that is discharged from the underdrain pipes is monitored at least weekly for flow rate, pH, and CN WAD in <br />accordance with the AM-10 approved monitoring plan. As the underdrain ponds have demonstrated to be a <br />wildlife attractant, the Division will continue to evaluate sampling results and any wildlife observations provided <br />by CC&V to determine whether additional monitoring and/or wildlife safeguards are warranted. <br /> <br />Construction workers were observed beginning the final installation of the underdrain sump (see Photo 2). <br /> <br />AGVLF Inspection: <br />As part of the Division’s oversight of the AGVLF with respect to long term stability, the placement of <br />agglomerated tailings was observed during this inspection. Mr. Hanks explained haul trucks are loaded with ore <br />at the Load Out Bin (LOB), then driven to the agglomerated tailings stockpile (see Photo 3) above the high grade <br />mill where a loader adds 2 to 3 buckets of agglomerated tailings into the haul truck. Photo 4 shows a haul truck <br />dumping a load of crushed ore combined with agglomerated tailings on the Phase IV AGVLF pad. Mr. Hanks <br />indicated the agglomerated tailings are not placed where the exposed slope would be a final outslope and that <br />the goal is to place the agglomerated tailings within the interior of the final pad configuration. He also stated a <br />dozer is typically used to mix the crushed ore and tailings. However, that was not the case with this load. Photo <br />5 shows the dumped agglomerated tailings on a slope that will ultimately be in the interior of the AGVLF pad <br />has the final haul road is narrowed. Mr. Hanks indicated the mine is discussing alternatives to agglomerating <br />the tailings from the high grade mill and will keep the Division apprised of their plans. A corrective action <br />addressing the vibration from the high grade mill drum agglomerator is due to the Division January 19, 2016. <br /> <br />The tension cracks in the haul road on the north side of the AGVLF first observed during the Division’s October <br />2015 inspection are shown in Photo 6. Mr. Hanks explained that the inspection by geotechnical specialists <br />planned for a few days after the Division’s October inspe ction was delayed due to a snow storm. CC&V will <br />provide the Division with a report addressing the observed tension cracks as soon as it is completed. <br />
