Please find listed below <br /> information related to this permitted activity: <br /> 1) Brief description of operation, purpose, and chemical/hazardous nature of the <br /> processed product/fresh ore mix being transported (be sure of include the volume of <br /> ore, equipment to be used, and description of containment); <br /> Fresh ore is currently being hauled from the load out bin on the certified lined area of Phase IV <br /> of the Arequa Gulch Valley Leach Facility (AGVLF)to the certified lined Process Solution <br /> Storage Area(PSSA) of the Squaw Gulch Valley Leach Facility (SGVLF) and on Phase 1 to <br /> prepare haul roads. Prior to the approval of the Cripple Creek and Victor Gold Mining Company <br /> Squaw Gulch Valley Leach Facility Quality Assurance Monitoring and Test Results Phase 1 <br /> (9,450 to 9,550 Bench) Final Report Teller County, Colorado (Cent. Report) submitted to the <br /> Colorado Division of Reclamation, Mining and Safety on 13 October 2015; CC&V understands <br /> that any fresh ore placed above the 9,450" elevation is at the owners risk and that CC&V cannot <br /> leach or place processed product that has under gone cyanide leach on Phase 1 until the Cert. <br />
