Adequacy Review No. 2 <br />Dear Mr. Nettleton: <br />The Colorado Division of Reclamation, Mining, and Safety (Division) received MR287 on April 13, <br />2015. The application was called complete for review on August 25, 2015. The Division received your <br />response to the first adequacy review letter on September 24, 2015. The following is a list of items <br />identified in the first adequacy review. For each item, your response to the item is cited and the <br />Division's response follows. If an item is resolved, that will be indicated. If additional information is <br />needed, it will be discussed. <br />Proposed revised page 2.04-9.1 is missing information from the currently approved page. The <br />section discussing the 15 -Left rock dust tank installation and 16 -Left utility borehole is missing <br />currently approved information regarding the 5MN Cement Borehole and Pipeline from MR283 <br />and MR288. Please revise this page to include the missing information. <br />a. TC's response: The referenced page has been reviewed and revised to incorporate the <br />currently approved permit text. Copies of the revised pages are provided with these <br />responses for replacement in the PAP. <br />b. Division's response: Once the Offset Drilling Zones have been surveyed for cultural <br />resources, if archeological and/or cultural resources are identified as being eligible to be <br />listed in the National Register of Historic Places (NRNP), please updates section 2.04.4 <br />of the permit to indicate any sites found to be eligible for listing in the NRNP. If any <br />sites are found meeting this criteria, section 2.05.6(4) of the permit will need to be <br />updated to indicate which sites have been identified as eligible for listing on the NRHP <br />and a description of measure to be used to minimize or prevent impacts to these <br />resources will need to be discussed. <br />2. Proposed revised page 2.05-17.3 is missing information from the currently approved page. The <br />top of the page is missing information regarding field operations for the 2012 WMD seismic <br />survey. Given this, information from the currently approved page 2.05-17.4 has moved up on the <br />document and is now on the revised page 2.05-17.3.
