Compliance With Mine Plan N - Support Facilities Not On-site <br />N - Other N - Special Categories Of Mining <br />N - Processing Waste N - Topsoil <br />COMMENTS <br />This was a complete inspection of the North Thompson Creek Mine conducted on Wednesday the 16th of <br />September, 2015. The inspection was conducted by Clayton Wein accompanied by Jim Stark, both of CDRMS. <br />The site is inactive, therefore there was no company representative present for the inspection. The weather was <br />clear and about 70 degrees fahrenheit. The ground conditions were mostly dry with areas that were damp from <br />rain earlier in the day. <br />AVAILABILITY OF RECORDS — Rule 5.02.4(1): <br />The Records are kept at the Garfield County Courthouse located in Glenwood Springs. The records were <br />in good condition. Deficiencies were identified with the records: The insurance certificate in the records has <br />expired and needs to be replaced with the current certificate. The Rill and Gully Survey was for the <br />previous year. The records need to be updated with the 2015 survey. The NPDES permit had expired in <br />January of 2015 and the NPDES records did not include the second quarter of 2015. These need to be <br />updated in the records. The quarterly pond inspections included the first quarter of 2015 but did not have <br />the second quarter inspections. Please see the included Availability of Records form at the end of the report for <br />more details <br />HYDROLOGIC BALANCE - Rule 4.05 <br />Drainage Control 4.05.1, 4.05.2, 4.05.3; Siltation Structures 4.05.5, 4.05.6; Discharge Structures 4.05.7, 4.05.10; <br />Diversions 4.05.4; Effluent Limits 4.05.2; Ground Water Monitoring 4.05.13; Surface Water Monitoring 4.05.13; <br />Drainage — Acid and Toxic Materials 4.05.8; Impoundments 4.05.6, 4.05.9; Stream Buffer Zones 4.05.18: <br />- During the time of the inspection both the no. 1 and no. 3 discharge pipes were discharging into the Long Pond. <br />Approximately 5 to 10 gallons per minute was discharging from the mine no. 1 pipe and just a small trickle from <br />the No. 3 pipe. (Photo 1) <br />-The Long Pond was well vegetated and the embankment looked stable.
