PERMIT #: M-1981-086 <br />INSPECTOR'S INITIALS: PSH <br />INSPECTION DATE: September 16, 2015 <br />The site is permitted for 37.30 acres. The site was active during the inspection. Stockpiled road base material <br />was being hauled from the site by Grand County Department of Road and Bridge. Grand County is contact <br />mining the site for NCWCD. Two stockpiles of road base and a stockpile of rock were observed on the pit <br />floor. The highwall is progressing to the northwest and was estimated at 15 feet in height. The highwall was <br />temporarily sloped at a 3H:1V grade. Topsoil and overburden material were stripped ahead of the highwall. <br />The approved post mining land use for the site is general agriculture. No reclamation activities have begun at <br />the site. Mr. Struble mentioned reclamation of the southeast portion of the site, except for the access road, <br />would begin soon. No noxious weeds were observed during the inspection. Overburden and topsoil <br />stockpiles were observed in the southwest corner of the site. The topsoil and overburden stockpiles were well <br />vegetated and appeared stable. <br />The mine sign and boundary markers were observed as required by Rule 3.1.12. The site is delineated by a <br />fence on the north and west sides and metal T -posts on the south and east sides. The northern permit <br />boundary, current highwall location and the new access road were GPS surveyed by the Division with a <br />Trimble Juno 3B. A map of the GPS survey results is attached. <br />The erosion and stormwater problems noted in the previous inspection report have been resolved by the <br />ending of the hay field irrigation northwest of the site. The pit floor was dry and no erosion features were <br />observed during the inspection. <br />The Division currently holds a financial warranty in the amount of $36,100, which is adequate to reclaim the <br />site based on this inspection. <br />As cited as a problem requiring corrective action on Page 1, the Operator has constructed a new access road in <br />the middle of the site. The access road is not included in the approved mine and reclamation plans for the <br />site. The Operator must submit a Technical Revision, with the required $216 revision fee, to update and clarify <br />the approved mine and reclamation plans and maps to reflect the new access road. The Operator must <br />demonstrate no new affected acreage was necessary to construct the road as part of the technical revision. <br />An amendment to the current permit may be required if the construction of the road resulted in an increase in <br />affected acreage at the site. <br />Photographs taken during the inspection are attached. <br />Page 2 of 5 <br />
