OPERATORS <br />SHOULD READ THE FOLLOWING PAGES CAREFULLY IN ORDER TO ASSURE COMPLIANCE <br />WITH THE TERMS OF THE PERMIT AND APPLICABLE RULES AND REGULATIONS. If a <br />Possible Violation is indicated, you will be notified under separate cover as to when the Mined Land <br />Reclamation Board will consider possible enforcement action. <br />INSPECTION TOPIC: Roads <br />PROBLEM/POSSIBLE VIOLATION: Problem: A new access road has been constructed by the Operator. The <br />current mine and reclamation plans need to be updated and clarified pursuant to C.R.S. 34-32.5-112 (1)(c)(VI) <br />and C.R.S. 34-32.5-116 (1) for the new access road. The Operator must provide sufficient information to <br />describe and identify the new road and provide sufficient information to describe or identify how the Operator <br />intends to reclaim the road. <br />CORRECTIVE ACTIONS: The Operator must submit a Technical Revision, with the required $216 revision fee, to <br />update and clarify the approved mine and reclamation plans and maps to reflect the new access road by the <br />corrective action date. The Operator must demonstrate no new affected acreage was necessary to construct <br />the road as part of the technical revision. An amendment to the current permit may be required if the <br />construction of the road resulted in an increase in affected acreage at the site. <br />CORRECTIVE ACTION DUE DATE: 10/22/15 <br />OBSERVATIONS <br />The Borrow Area C site was inspected by Peter Hays with the Division of Reclamation, Mining and Safety <br />(Division/DRMS) as part of the Division's monitoring inspection program. Mr. Jim Struble and Mr. Noble <br />Underbrink with Northern Colorado Water Conservancy District (NCWCD) were present during the inspection. <br />The site was previously inspected by the Division on July 27, 2011. <br />Page 1 of 5 <br />
