Everist, Inc. respectfully requests a Technical Revision to the reclamation plan for the Fort Lupton <br />Sand and Gravel Mine, DRMS Permit # M-1999-120. We have included the Request for Technical <br />Revision (TR) Cover Sheet (form) and Check #1846 for $216.00 to cover the Technical Revision fee. <br />As we noted in our 2012 amendment, there could be future changes to the reclamation plan due to <br />changes with oil and gas facilities. For this Technical Revision, we are revising the reclamation plan <br />slightly, making minor changes to three reservoirs, and connecting two of them. <br />Meadows North and Meadows South will now be connected as Meadows Lake. <br />We are making minor changes to the reclamation configuration of Meadows North and Meadows South. <br />The divider between Meadows North and South has been removed, and the two reservoirs are now one, <br />to be called Meadows Lake. This change has come about due to the 2013 flood, which washed out part <br />of the divider between the two lakes. Taking the divider out is an interior change to the reservoirs and <br />does not affect the single slurry wall liner which was constructed around the outside of both lakes. <br />L.G. Everist worked together with Foundation Energy and Encana Energy, which each had one gas well <br />along the divider in Meadows North and South. Foundation Energy and Encana Energy capped and <br />removed their gas wells, Foundation Energy Gas Well/Pump Unit - Amaco-Ewing #1 and Encana Energy <br />Gas Well - Harry Ewing #1. The corresponding gathering lines and facilities were also removed and/or <br />re-routed.
