July 23,2015 C-1981-035/King Coal Mine RAR <br /> At the King II sediment pond the top lip of the outlet pipe for the primary spillway was slightly <br /> warped, and should be straightened. <br /> INSPECTION.- <br /> SIGNS AND MARKERS Rule 4.02 <br /> o Mine Identification signs Permit Boundary markers and surface disturbance markers identified <br /> during this inspection were installed as required throughout the areas visited. <br /> o Topsoil stockpiles observed were clearly marked with identifying signs. <br /> ROADS Rule 4.03 <br /> Construction 4.03.1(3) 14.03.2(3); Drainage 4.03.1(4) I4.03.2(4); Surfacing and Maintenance <br /> 4.03.1(5) and (6) 14.03.2(5) and (6); Reclamation 4.03.1(7) 14.03.2(7): <br /> o All roads in and around the facilities area were stable. The main haul road at King II was in good <br /> condition. No dust was being generated. <br /> o East Clearwater Ditch along the main haul road changes gradient near the mine entrance. The <br /> ditch is stable with no soil moving and no evidence of erosion (Photo 1). There is no need to add <br /> riprap or other stabilizing material at this knickpoint. <br /> SUPPORT FACILITIES-Rule 4.04: <br /> o Volumes of coal at both stack tubes (91 and#2) were contained in the designated areas. <br /> Flags were visible and easy to identify. <br /> o There were a number of haul trucks waiting to be loaded with coal and some trucks were <br /> loaded during the inspection. <br /> HYDROLOGIC BALANCE-Rule 4.05 <br /> Drainage Control4.05.1, 4.05.2, 4.05.3;Siltation Structures 4.05.5, 4.05.6; Discharge <br /> Structures 4.05.7, 4.05.10; Diversions 4.05.4; Effluent Limits 4.05.2; Ground Water Monitoring <br /> 4.05.13; Surface Water Monitoring 4.05.13; Drainage —Acid and Toxic Materials 4.05.8; <br /> Impoundments 4.05.6, 4.05.9; Stream Buffer Zones 4.05.18: <br /> o The King II sediment pond held water and was not discharging. The primary and emergency <br /> spillways were clear(Photo 1). <br /> o The West Clear Water Ditch was free of debris, adequately rip rapped and appeared to <br /> be functioning correctly. The ditch was not transmitting water at the time of inspection. <br /> After an aerial survey in November of 2014, the ditch had been surveyed by a <br /> professional surveyor in March 2015 to assure it met design criteria. <br /> o There is an upgradient NPDS monitoring point at the Ute Mountain Tribal Building in <br /> Hay Gulch as well as a NPDS monitoring point at the discharge to the sediment pond. <br /> o The East Clear Water Ditch passes through a reclaimed area.
