Scope of Work <br />5 MN Borehole to 6 MN Pond <br />Buried Dewatering Line <br />07/28/15 <br />Contractor shall also provide for re -seeding of the topsoil during the late summer of 2015. The <br />required seed mixture is attached in table 53. <br />The contractor will also be responsible for the procurement and installation of the five culverts. These <br />culverts shall be 10' diameter by 10' high. One culvert will be located at the top of the 5MN borehole <br />while the others will be located at all the gate valve locations. When finished, each of the culverts <br />should be equipped with a lid and ladder. Please see attached photos and Figure 2 for the culvert <br />design. As -built design for culverts is to be close to that of the attached photos and Figure 2, and will <br />be verified by a TC representative. <br />The contractor will be responsible for the collection and disposal of all waste materials from the pipe <br />trimming and fusing process, including the disposal of any wood products supplied with the HDPE <br />pipe. The waste materials may be disposed of in any of the appropriate trash containers located on TC <br />mine property. <br />A site visit is strongly recommended. <br />TC Responsibilities <br />1. Obtain all required environmental permits and approvals. <br />2. Provide surveyed route of line to be buried. <br />Provide all HDPE pipe and associated tees, flanges and miscellaneous fittings and monitoring <br />devices. <br />4. Provide on site hazard training necessary for contract personnel to operate on the surface areas. <br />Contractor's Responsibilities <br />1. Provide all materials, equipment, tools, services, insurance, engineering design, labor, and <br />supervision for the turnkey design and construction for this project as required by these <br />specifications and parameters. <br />Contractor shall furnish a list of equipment available and anticipated to be used on the project. <br />Contractor shall furnish a list of supervisory, technical and management personnel to be associated <br />with the project. <br />2. All contractor personnel on the site shall have completed the required 30 CFR Part 48.5, 24-hour <br />surface training. Copies of the MSHA Form 5000-23 training certificate, for all persons assigned <br />Page 4 <br />
