PERMIT #: M-1995-026 <br />INSPECTOR'S INITIALS: TOD <br />INSPECTION DATE: February 18, 2015 <br />OBSERVATIONS <br />This normal routine monitoring inspection was conducted by Tyler O'Donnell of the Division of Reclamation, <br />Mining and Safety (Division). Custer County, the Operator, was represented by Roger Squire and Rusty <br />Christensen during the inspection. <br />The Gravel Pit M-93 is located in Custer County approximately 4.5 miles southeast of Westcliffe Colorado. The <br />Gravel Pit M-93 is a 40.3 -acre 112c Construction Materials Reclamation Permit, which was issued in August of <br />1995. The primary commodity mined at the site is sand and gravel. The approved post -mining land use is <br />Rangeland. The mine site was surrounded by the following land uses: industrial/commercial, residential, <br />agricultural, rangeland, and mining. <br />No problems or violations were noted during the inspection. During the inspection the sky was clear and the <br />ground had very partial snow cover. Mining equipment was present during the inspection however the site was <br />not operational during the inspection. It appears that the last mining activities were progressing from south to <br />north. At the present time it appears that the mining operation has affected approximately 15 acres. There are <br />portions of the site in various stages of reclamation. The Operator has been doing adequate concurrent <br />reclamation. The Operator's representative stated that the pit is used frequent use for road base throughout the <br />county. <br />Acid And Toxic Materials: <br />There was an onsite fuel storage tank in the southwest corner of phase 1. The fuel tank appeared to have adequate <br />110% spill containment. No evidence of a petroleum spill was observed during the inspection. <br />(Note: Secondary containment structures shall consist of an impermeable containment which could contain all <br />contents of the tanks and various containers (when full) plus 10% of the total capacity.) <br />Backfilling and Grading: <br />The majority of the pit slopes were graded to approximately 3H:1 V or flatter. An active mining face was observed <br />along the northern and eastern portion of phase 1 progressing into phases 3 and 4. The active mining face was <br />stepper that 3H:IV. <br />Hydrologic Balance: <br />The pit daylight to the south. The bottom of the pit was dry. There was no evidence of excessive erosion. There <br />appeared to be no significant impacts to the prevailing hydrologic balance. <br />Gen. Compliance With Mine Plan: <br />The operation appeared to be following the approved mine plan. All mining activity appeared to be within the <br />approved permit boundary. <br />Reclamation Success: <br />The Operator has successfully graded the western slopes of phases 1 and 4 after being mined. The Operator has <br />been successfully completing concurrent reclamation. The final graded slopes along the western portion of the <br />pit are stable and have begun to establish desired vegetation. <br />Reve etg_ ation: <br />No noxious weeds were observed during the inspection. There was some plant growth on the stockpiles and <br />throughout the disturbed area. The growth in the disturbed area appeared to be volunteer vegetation, comprised <br />of grasses and a few annual weeds. <br />Page 2 of 5 <br />
