PERMIT #: M-1982-086 <br />INSPECTOR'S INITIALS: MAC <br />INSPECTION DATE: June 29, 2015 <br />OBSERVATIONS <br />The inspection was conducted by Michael Cunningham of the Division of Reclamation, Mining and Safety <br />(Division). Mark Martin of Alpine Rock Company was present on behalf of the Operator. The Fairplay Site is <br />located south of the town of Fairplay alongside the Middle Fork of the South Platte River. The operation is <br />permitted for 144 acres and the post -mining land use is rangeland. The site contains substantial piles of <br />historic dredge tailings, which are processed as a construction material and are primarily used on road <br />construction projects. <br />The permit sign was posted at the site entrance and the boundary of the affected area was adequately <br />delineated. The site was not operating at the time of the inspection. However, the Operator was in the <br />process of bringing in equipment to set up the asphalt batch plant which should be set up and operating by <br />the middle of July 2015. The Fairplay site is authorized to operate an asphalt batch plant as well as a concrete <br />batch plant; all structures associated with the batch plants are to be portable. <br />The Mining Plan for the Fairplay Site consists of mining the dredge tails down to the natural ground surface. <br />The existing plant and stockpile area consists of leveled ground that is approximately 15 acres in size. To date, <br />the operation has removed approximately 12 acres of the historic dredge tailings. The stockpile area <br />contained numerous stockpiles of crushed material; no crushing equipment was present. To date, final <br />reclamation has not been performed on any portion of the site. <br />A review of the permit file found that the Division does not have copies of the Mining Plan or Reclamation <br />Plan Maps which conform to the requirements of Construction Material Rules 6.2.1(2), 6.4.3, and 6.4.6. This <br />has been cited as a problem and will require corrective action by the Operator. If the Operator has copies of <br />the originally approved Maps, they may be submitted to the Division for inclusion into the permit file. <br />Otherwise, a new set of maps will need to be submitted through a Technical Revision. Additional details <br />regarding the required corrective action may be found on the first page of this report. <br />The existing financial warranty is $141,700. The last financial warranty calculation was performed in 2010. The <br />Division has confirmed that the financial warranty remains adequate to complete reclamation of the site. <br />This concluded the inspection. <br />Page 2 of 4 <br />
