OPERATORS <br />SHOULD READ THE FOLLOWING PAGES CAREFULLY IN ORDER TO ASSURE COMPLIANCE <br />WITH THE TERMS OF THE PERMIT AND APPLICABLE RULES AND REGULATIONS. If a <br />Possible Violation is indicated, you will be notified under separate cover as to when the Mined Land <br />Reclamation Board will consider possible enforcement action. <br />INSPECTION TOPIC: Availability Of Records <br />PROBLEM/POSSIBLE VIOLATION: Problem: The Division does not have copies of the Mining Plan or Reclamation <br />Plan Maps which conform to the requirements Construction Material Rules 6.2.1(2), 6.4.3, and 6.4.6. <br />CORRECTIVE ACTIONS: The Operator may submit copies of the originally approved Maps which conform to the <br />requirements of Rule 6.2.1(2), 6.4.3, and 6.4.6 for inclusion into the permit file. Otherwise, a new set of maps <br />will need to be submitted through a Technical Revision. <br />CORRECTIVE ACTION DUE DATE: 8/14/15 <br />Page 1 of 4 <br />
