17. Please revise the Mining Plan Map to include the following: <br />a. A labeled permit boundary. The permit boundary shall correspond to the proposed 2.1 <br />acre permit boundary. <br />b. Label the names of owner(s) of record of the surface of the affected area and of the <br />land within 200 feet of the affected area and identify the owner of the substance to be <br />mined. <br />c. Outline and label all existing and proposed adits. <br />d. Show the name of the applicant. <br />e. Give date prepared. <br />f: Map must be signed by a registered land surveyor, professional engineer, or other <br />qualified person. <br />g. For ease of use the Division requests the applicant change the units on the scale bar <br />from metric to standard. <br />The above requested changes have been addressed and can be observed on the Mining Plan Map <br />attached hereto. <br />18.
