Compliance With Mine Plan N - Support Facilities Not On-site <br />N - Other N - Special Categories Of Mining <br />N - Processing Waste N - Topsoil <br />COMMENTS <br />This was an aerial inspection of the Terror Creek Loadout conducted by Jim Stark of the Colorado Division of <br />Reclamation, Mining and Safety. Harry Ranney, also with the Division, took the aerial photographs. Since this <br />was an aerial inspection, no company representative was present. The Terror Creek Loadout has basically stopped <br />operating and currently being (at least partially) reclaimed. The weather was sunny and there was some water on <br />the ground from recent rains. <br />HYDROLOGIC BALANCE - Rule 4.05: Drainage Control 4.05.1, 4.05.2, 4.05.3; Siltation Structures 4.05.5, <br />4.05.6; Discharge Structures 4.05.7, 4.05.10; Diversions 4.05.4; Effluent Limits 4.05.2; Ground Water Monitoring <br />4.05.13; Surface Water Monitoring 4.05.13; Drainage — Acid and Toxic Materials 4.05.8; Impoundments 4.05.6, <br />4.05.9; Stream Buffer Zones 4.05.18: <br />- There was some water in the sediment pond but it appeared to be several feet below the elevation of the primary <br />spillway. The embankment was well vegetated and appeared to remain stable. No erosional problems were noted <br />on the pond embankment or inslopes. <br />- The dugout pond was dry at the time of the inspection. The pond area was well vegetated and stable and no <br />erosional problems were noted. <br />GENERAL MINE PLAN COMPLIANCE: <br />- The Terror Creek Loadout was not operational at the time of the inspection and reclamation of the facilities has <br />commenced. The site is in compliance with the mine plan and permit. <br />- There was a drill rig sitting at the bottom of the east light -use road along the railroad tracks. The rig will drill <br />three alluvial wells, which were permitted in Minor Revision 28, Approved on 13 April 2015. <br />ROADS — Rule 4.03: Construction 4.03.1(3)/4.03.2(3); Drainage 4.03.1(4)/4.03.2(4); Surfacing and Maintenance <br />4.03.1(5) and (6)/4.03.2(5) and (6); Reclamation 4.03.1(7)/4.03.2(7): <br />- The main haul road is a paved road. The road was well maintained and stable and no problems were noted in the <br />photographs. The truck loop is a dirt road.
