May 19, 2015 C-1981-019/Colowyo Coal Mine RDZ <br />EXPLOSIVES — Rule 4.08 <br />Distance Prohibitions 4.08.4; Warnings 4.08.4; Control of Adverse Effects 4.08.4: <br />The inspection entailed a check of records of blasting, including the detailed records for 10 recent shots (in <br />Chip Brown's office) as well as monitoring records (in the Administrative Building). The dates of the blasts (all in <br />2015) that were checked are: April 21, 23, 27, and 30 and May 4, 6, 7, 11, 14, and 18. The monitoring records for <br />these dates did not show that any readings corresponded with the times of blasts. <br />Overall, the records were in order, but some minor errors were found by the OSM inspectors. For the March 18 <br />blast, the direction to the nearest dwelling was missing. Changes were made to the blasting report form by <br />Colowyo during the inspection to address other OSM comments. The new forms reflect the following changes: <br />The names of the nearby residences have been updated on the blasting report form. Cannon (Cannon Family <br />Partnership) is the new name replacing Stevens. <br />The blasting report form now includes the name of the leadman (foreman at the site) and the blaster in charge <br />(Chip Brown), as well as their license numbers. Mr. Brown will sign the forms. <br />A field has been added to describe the type of delay that is used for each blast. This information was on the older <br />forms (Unitronics is both the initiation system and the delay system) but was not clear. <br />In the blast monitoring records, Kurtis Blunt added a specifications sheet for the Colowyo Instantel units. This <br />was done so that instrument sensitivity is provided in the records. <br />Rob Zuber used an Excel spreadsheet to check the March 18 blast to confirm that the maximum weight per 8 ms <br />delay is in compliance. It was found to be in compliance. <br />Both the individual shot records and the monitoring records included records for the past 36 months (or more). <br />Tony Tennyson indicated that there have been no requests by the public for records in the last 36 months. <br />During the inspection, Kurtis Blunt re -installed the Intstantel units at the Ott and Cannon locations. They <br />appeared to be installed correctly. The microphone at the Cannon location is directed toward the South Taylor <br />Pit. The microphone at the Ott location is pointing to the northwest, but the pit is west of this site. Per Ken <br />Eltschlager of OSM (phone conversation of May 22, 2015), this should be sufficient as long as there are no <br />obstructions. <br />HYDROLOGIC BALANCE - Rule 4.05 <br />Drainage Control 4.05.1, 4.05.2, 4.05.3; Siltation Structures 4.05.5, 4.05.6; Discharge Structures 4.05.7, 4.05.10; <br />Diversions 4.05.4; Effluent Limits 4.05.2; Ground Water Monitoring 4.05.13; Surface Water Monitoring 4.05.13; <br />Drainage — Acid and Toxic Materials 4.05.8; Impoundments 4.05.6, 4.05.9; Stream Buffer Zones 4.05.18: <br />The following ponds were inspected: Gossard, Prospect, Gulch A, Streeter, and Work Area. The <br />following were discharging: Gossard, Prospect, and Streeter.
