May 18, 2015 C-1981-035/King Coal Mine MLT <br />• The area surrounding the pad at Coal Stack Tube 92 had been cleared of the coal that was <br />found during the April 21, 2015 inspection and for which a violation was issued. The <br />Division finds that the work has been completed (Photo 1) in a satisfactory manner, and the <br />violation has been abated. <br />• The Division suggested that a line of stakes could be placed in the ditch flowline around <br />Stack Tube 42, to prevent an event of this type from occurring again. <br />• At Stack Tube 41 (Photo 2), the stockpile of coal on hand was fairly small. The safety berm <br />around the pad perimeter was in place, with coal spilling over into the collection ditch at a <br />few locations. <br />• At King I, a 40 cu. yd. dumpster was on site (Photo 3), and had been filled with the <br />miscellaneous debris noted in the OSM inspection report for April 21, 2015. <br />HYDROLOGIC BALANCE - Rule 4.05 <br />Drainage Control 4.05.1, 4.05.2, 4.05.3; Siltation Structures 4.05.5, 4.05.6; Discharge <br />Structures 4.05.7, 4.05.10; Diversions 4.05.4; Effluent Limits 4.05.2; Ground Water Monitoring <br />4.05.13; Surface Water Monitoring 4.05.13; Drainage Acid and Toxic Materials 4.05.8; <br />Impoundments 4.05.6, 4.05.9; Stream Buffer Zones 4.05.18: <br />• Ground conditions at the surface facilities were generally damp due to the recent rains, with <br />occasional puddles present. <br />• All three sediment ponds were holding water, but none were at a level where discharge <br />would occur. <br />• Ditches and culverts were inspected at King II, and found to be clear and in good working <br />order. The East and West Clear Water diversion ditches were in good repair. <br />• At King I, maintenance on the system of ditches and culverts (locations needing attention <br />were identified in the Division's inspection report for March 31, 2015) had been performed. <br />• There are two wells at the King I facilities: The Smith well, located inside the old pump <br />house (Photo 4), and the Right well, on the south side of the sales office building (Photo 5). <br />TOPSOIL Rule 4.06 <br />Removal 4.06.2; Substitute Materials 4.06.4(4); Storage and Protection 4.06.3; Redistribution <br />4.06.4: <br />• The two topsoil stockpiles at King II were well vegetated and protected from any loss of the <br />resource. <br />EXCESS SPOIL and DEVELOPMENT WASTE Rule 4.09 <br />Placement; Drainage Control; Surface Stabilization: <br />• At King I, loads of development waste have been dumped on top of the refuse pile and in the <br />temporary storage area near the location of the old tipple (Photo 6).
