April 23, 2015 C- 1981 - 018 /Deserado Mine ZTT <br />well vegetated and was sufficient for erosion control. Two silt fences were additionally included with this site <br />for extra erosion control. Sediment deposition was observed above the silt fences and each fence was in <br />working order. All other sites inspected were in compliance based upon substantial vegetative growth or <br />surface roughening for erosion control. No erosion or sediment leaving any of the sites was observed. <br />SEDCAD demonstration outline 93 is the demonstration used for all drill pad sites associated with the D -seam <br />mining. As early noted, drill pad construction practices outlined in this demonstration are no longer used on <br />site. Stated in Illustration 44 for Typical Borehole, Exploration and Shaft Pads, this demonstration is for a <br />worse case location on a steep side hill. The disturbed area is 0.43 acres and has a perimeter berm and ditch <br />surrounding the area and separating it from any undisturbed runoff. Berms will be breached at low points to <br />prevent impound men and either a porous rock outlet structure or silt filter fabric will be used. <br />Five degas pad sites associated with demonstration 93 were inspected. All of the pad sites that were reclaimed <br />were done in the late 1990's. Vegetation was very established in each of the pads. Berms were stable and all <br />outlet structures were in working order. No signs of sediment deposition along the low points of each berm <br />were observed. No erosion leaving sites was encountered at any of the pads. Borehole LW1 -8 was the only <br />pad site inspected related to demonstration #3 that is still active. No issues were observed with this specific <br />site regarding hydrologic balance. <br />Silt fence located at LWB 10 -2. <br />GENERAL MINE PLAN COMPLIANCE: <br />LW104 with berm. <br />Reclamation methods for sealing drill holes are outlined in Section V.F - Sealing of Mining Openings and <br />Drill Holes. Drill hole plugging typical can be found in the same section on page V -20. All sealed drill holes <br />were inspected and found in compliance and in accordance with Deserado's permit. <br />Excess pipe encounter at pad site LW1 -8 needs to be removed and stored in an approved area. <br />Number of Partial Inspection this Fiscal Year: 6 <br />Number of Complete Inspections this Fiscal Year: 3 <br />Page 6 of 9 <br />
