April 21, 2015 C- 1981- 035/King Coal Mine MLT <br />SIGNS AND MARKERS Rule 4.02: <br />• New mine identification signs had been posted at the entrances to the King I and King II <br />surface facilities (Photo 3). <br />• Topsoil stockpiles at King II were posted as required, and surface disturbance markers were <br />in place surrounding the King I and King II facilities. <br />ROADS Rule 4.03 <br />Construction 4.03.1(3) / 4.03.2(3); Drainage 4.03.1(4) / 4.03.2(4); Surfacing and Maintenance <br />4.03.1(5) and (6) 14.03.2(5) and (6); Reclamation 4.03.1(7) / 4.03.2(7): <br />As we approached the mine from the east, we observed that the gravel- surfaced portion of <br />County Road 120 (CR 120) had been recently sprayed with water. We were subsequently <br />informed by mine personnel that GCC had reached an agreement with La Plata County, and <br />had been granted permission to spray the county road as needed to control the dust. <br />• On CR 120, we found ourselves in line behind a queue of perhaps half a dozen highway haul <br />trucks that were returning to King II to be loaded with coal for delivery to customers. The <br />trucks were traveling approximately 25 mph (the posted speed limit is 35 mph). <br />• The main haul road into the King II facilities was graded, had been sprayed with water, and <br />was in fair condition. Access road surfaces were also damp. <br />SUPPORT FACILITIES - Rule 4.04: <br />• The height and volume of coal stockpiled at Stack Tubes 41 (Photo 4) and 42 (Photo 5) was <br />significantly reduced compared with what has been observed on previous inspections. The <br />pile footprints were still enlarged, however. <br />• Processed coal stockpiled at Stack Tube #2 had covered the ditch on the pad's western side, <br />and had encroached upon the adjacent undisturbed area (Photos 6 and 7). A Notice of <br />Violation (CV- 2015 -001) is being issued to the operator. <br />• Modifications have been completed on the mine's ventilation intake fan (Photo 8). A new <br />muffler segment has been inserted into the duct, and the elbow now points skyward rather <br />than into the shotcrete -lined cliff face. <br />• Maintenance on the belt portal bench had been performed since the Division's March 2015 <br />inspection. Material that had accumulated as a "crown," west of the conveyor, had been <br />shaved off to restore the original elevation. There remains some work to be done on the <br />adjacent berm — rocks and plowed -up material appear poised to move down the slope, and <br />should be pulled back toward the bench or relocated to a more suitable location. <br />• The three -sided petroleum storage building was in order, and the sump behind the building <br />(there in case of a tanker spill) was emptied of sediment. <br />HYDROLOGIC BALANCE - Rule 4.05 <br />Drainage Control 4.05.1, 4.05.2, 4.05.3; Siltation Structures 4.05.5, 4.05.6; Discharge <br />Structures 4.05.7, 4.05.10; Diversions 4.05.4; Effluent Limits 4.05.2; Ground Water Monitoring <br />4.05.13; Surface Water Monitoring 4.05.13; Drainage Acid and Toxic Materials 4.05.8; <br />Impoundments 4.05.6, 4.05.9; Stream Buffer Zones 4.05.18: <br />Number of Partial Inspection this Fiscal Year: 8 <br />Number of Complete Inspections this Fiscal Year: 4 Page 3 of 13 <br />
