Compliance With Mine Plan <br />N - Support Facilities Not On -site <br />N - Other <br />N - Special Categories Of Mining <br />N - Processing Waste <br />R - Topsoil <br />COMMENTS <br />This was a partial inspection of the New Horizon Mine conducted by Brock Bowles, Robin Reilley and Tabetha <br />Lynch of the Colorado Division of Reclamation Mining and Safety. Frank Ferris of Western Fuels- Colorado was <br />present for the entire inspection. The weather was sunny, warm and very windy. The ground conditions were dry. <br />HYDROLOGIC BALANCE - Rule 4.05 <br />Drainage Control 4.05.1, 4.05.2, 4.05.3; Siltation Structures 4.05.5, 4.05.6; Discharge Structures 4.05.7, 4.05.10; <br />Diversions 4.05.4; Effluent Limits 4.05.2; Ground Water Monitoring 4.05.13; Surface Water Monitoring 4.05.13; <br />Drainage — Acid and Toxic Materials 4.05.8; Impoundments 4.05.6, 4.05.9; Stream Buffer Zones 4.05.18: <br />The Pond 001 discharge flume was out of level in both directions (photos 1 & 2). The flume needs to be <br />re -set so it is level. Cattails are starting to sprout in the Pond 001 discharge channel. The cattails should be <br />removed before they can interfere with the channel flow. <br />Pond 001 embankment slopes were measured using a TruePulse 360R Laser Range Finder with an inclinometer. <br />The Pond- 001 As- Built, Appendix 2.05.3(4) -6 of the permit, shows the slopes as 3h: Iv or 33 %. The south side of <br />the pond is incised on the east end, has a 9.5 foot embankment in the middle and is also incised on the west end. <br />The inside embankment slopes were measured in 5 places from east to west as 30 %, 34 %, 41 %, 37% and 38 %. <br />The north side of the pond is completely incised and was measured in 5 places from west to east as 33 %, 32 %, <br />31 %, 25% and 34 %. The northeast corner was measured at 43% which is also completely incised. There are no <br />signs of seepage on the outslopes of the south embankment and no signs of significant erosion (i.e. rills /gullies). <br />The Pond 001 slopes appear to be stable and conform to the specification as out lined in Appendix 2.05.3(4) -6 of <br />the permit. <br />Pond 001 contained water but was not discharging. The Pond 001 east ditch was mostly clear but had a few piles <br />of tumble weeds (photo 3). Tom Frye said he removed all tumble weed piles the previous day but they have piled <br />up again because it has been really windy during the inspection and the previous 24 hours. <br />Number of Partial Inspection this Fiscal Year: 8 <br />Number of Complete Inspections this Fiscal Year: 3 <br />Page 2 of 4 <br />
