As required by Rule 6.4.12(1), all information to calculate the costs of reclamation must be <br />submitted and broken down into the various major phases of reclamation (i.e., quarry area, <br />processing/staging areas, storage areas, haul roads). I <br />6.4.13 Exhibit M — Other Permits and Licenses <br />42. A copy of an expired well permit was submitted with the application. Please specify if there <br />are any wells on site. If so, specify the intended use of the well and provide a copy of a valid <br />well permit. In addition, all wells are to be located on the Mining Plan Map. <br />i <br />43. A copy of an expired air quality permit was submitted with the application. Please indicate if <br />the Applicant has applied for a new air quality permit. In addition, please commit to <br />providing the Division with a copy of the air quality permit once is has been issued. <br />44. Please specify if the Applicant has obtained a Special Use permit from Larimer County. <br />6.4.14 Exhibit N — Source of Legal Right to Enter <br />45. The source of legal right to enter the property did not contain notarized signatures. Rule <br />6.4.14, requires that signed statement by landowners must be acknowledged by a Notary <br />Public. In addition, the statement should clearly indicate that the legal right to enter the <br />property is being granted for the purpose of mining and reclamation. Please submit a revised <br />legal right to enter the property. <br />6.4.15 Exhibit O — Owner(s) of Record of Affected Land (Surface Area) and Owners of <br />Substance to be Mined <br />No comment. <br />6.4.16 Exhibit P — Municipalities Within Two Miles <br />No comment. <br />6.4.17 Exhibit Q — Proof of Mailing of Notices to Board of County Commissioners and Soil <br />Conservation District <br />No comment. <br />6.4.18 Exhibit R — Proof of Filing with County Clerk and Recorder <br />No comment. <br />
