<br />L.G. Everist, Incorporated (aka L.G. Everist, Inc., Applicant, Operator) requests a Technical Revision JR)
<br />with a few minor changes to the Reclamation Plan as it is now approved. Please note that most of the
<br />reclamation activities have been completed at the mine, including a second hydro- seeding application on
<br />12/16/14 on areas that were disturbed.
<br />The minor changes are detailed below in paragraphs 1 -4 and are shown on the revised reclamation maps
<br />and table which are included in this submittal. Map Exhibit F - Reclamation Plan Map (As- Built) shows
<br />the changes as discussed below and represents the final As -Built plan. Figure E - Reclamation Plan
<br />(As- Built) shows the revisions on a color map. The reclamation timetable, Table E1: Reclamation
<br />Phases (As- Built) is also provided, which lists the changes in acreages of reclaimed areas.
<br />The operator is also submitting the Division of Reclamation, Mining and Safety's Technical Revision
<br />Request form and a check to cover the Technical Revision fee of $216.00 for a TR of a 112 permit.

<br />This change is requested to leave a larger part of the mined area as graded roads and graveled staging
<br />areas. Leaving a wider road and staging areas around the water reservoir facilitates future reservoir -
<br />related construction, operation and maintenance at the Star Water Resource. Therefore, the Applicant is
<br />revising the reclamation plan to leave additional graveled areas, as shown on the attached maps.
<br />The additional graveled roads and staging areas around the western reservoir are as follows: the 50 -foot
<br />wide slurry wall working platform that surrounds the reservoir (5.52 ac. ±); the access road in the
<br />southwest corner (0.23 ac. ±); and a road from the access road in the northeast corner to the pad (0.10
<br />ac. ±). Under the existing plan, a 25 -foot road was to remain on the slurry wall pad, so this is a minor
<br />increase. When the reservoir is developed by a future owner, there will be work going on from the outer
<br />edge of the pad to the proposed water level, so no revegetation will be needed on that area. The revised
<br />plan is to grade and gravel this road. The new roads and graveled areas will cover approximately 5.85
<br />acres, an increase of approximately 3.14 acres from the currently approved plan.
<br />There is also an area which lies south of the Concrete Batch Plant and oil well facilities in the central part
<br />of the mine, which had been designated as being vegetated. This area is being revised to be a graveled
