Tom Schlosser <br />February 3, 2015 <br />Page 2 of 6 <br />GENERAL: <br />Rule 62.1(2) that all maps, except the index map, must conform to the following criteria: <br />(a) show the name of the Applicant; <br />(b) must be prepared and signed by a registered land surveyor, professional engineer, or other qualified person; <br />(c) give the date prepared; <br />(d) identify and outline the area which corresponds with the application; <br />(e) and shall be prepared at a scale that is appropriate to clearly show all elements that are required to be <br />delineated by the Act. Also, a map scale, appropriate legend, map title, date and a north arrow shall be <br />included. <br />4. Maps provided in Exhibits C and F of the application do not meet all of these criteria. Please resubmit the <br />maps with all of the above criteria. <br />EXHIBIT C - Pre - mining and Mining Plan Map(s) of Affected Lands (Rule 6.4.3): <br />One or more maps may be necessary to legibly portray the following information: <br />The name and location of all creeks, roads, buildings, oil and gas wells and lines, andpower and communication <br />lines on the area of affected land and within two hundred (200) feet of all boundaries of such area. <br />5. The application includes a structure agreement for a power transmission line, which was observed along CR <br />49 during the pre - operational inspection, and will need to be indicated on one of the pre- mining maps in <br />Exhibit C. <br />The existing topography of the area with contour lines of sufficient detail to portray the direction and rate of slope <br />of the affected land. <br />6. The map submitted on page 4 -C of the application does not sufficiently portray the existing topography due <br />to faint contour lines and a small scale. In addition, this map does not show the existing topography of the <br />previously disturbed pit and overburden pile located on the proposed mine site. Please re- submit a map <br />clearly showing pre- existing topography of the site. The Division recommends transposing the contours <br />onto the Mine Plan Map on page 5 -C. <br />The total area to be involved in the operation, including the area to be mined and the area of affected lands (see <br />definition of 'Affected Land'). <br />7. Within the submitted Exhibit L — Reclamation Costs, the Applicant indicates that only 50 acres will be <br />disturbed. Please depict this 50 -acre "Affected Land" boundary on the Mine Plan Map. <br />The type of present vegetation covering the affected lands. <br />8. Please submit a new map, or update one that was previously submitted, showing the type(s) of present <br />vegetation within the permit boundary. <br />Show the owner's name, type of structures, and location of all permanent or man -made structures contained on the <br />area of affected land and within two hundred (200) feet of the affected land. <br />9. Please submit a new map, or update one that was previously submitted, showing information regarding <br />structures which include the well, county road, power transmission line, fences, ect. <br />
