The name and permit number of the mine and the name and address of the permittee are: Carbon Junction Mine (CDRMS permit number C- 1992 -080) Oakridge Energy, Inc. 4613 Old Jacksboro Highway Wichita Falls, Texas 76301 telephone (940) 322 -4772 2.04.13 (b). No additional acreage was disturbed in 2014. A minor amount of regrading to repair erosion due to heavy summer precipitation was undertaken at the Carbon Junction Channel and north of the Equipment Area. c). Backfilling and final grading of mine disturbance was completed in 2006. Backfilling, final grading and topsoiling of the sediment control system (Pond 001, Pond 002, and ditches) was completed in December 2008. No backfilling and grading occurred in 2014. d). Topsoil replacement was completed for the mine in 2006. Topsoil replacement on the final regraded areas of the reclaimed sediment control system was completed in December 2008. No topsoiling occurred in 2014. e). Revegetation was undertaken for the mine during 2006 with successful establishment of the perennial seed mix confirmed in 2008. Revegetation of the reclaimed sediment control system was undertaken in April 2009. No revegetation occurred in 2014.
