jurisdictional wetlands, the COE stated that this full area could not be in the permit because no <br />individual permit to disturb these wetlands was in place. The operator argued that no disturbance <br />would occur in the wetland portion until an individual permit was approved, however, the COE <br />insisted that the permits be identical, therefore, the DRMS permit had to be modified to only show <br />disturbance in that portion of the Atlas pile that did not have wetlands. For this reason, TR -02 was <br />submitted to reduce the size of the Atlas pile to avoid the wetlands which were present there. <br />This TR -02 necessitated changes to Appendix 6, which modified the sediment pond and ditch <br />designs for the Atlas area. Maps C -2, C -3, C -4, C -6, F -1, G -1, G -1B, G -2, G -5 were modified as part <br />of this revision. <br />TR -02 was approved in April of 2014. <br />TR -03 <br />During initial excavation to install the groundwater monitoring wells, it was found that two distinct <br />water zones were present at all three locations. After discussion with the Division, it was decided <br />that the three wells at the three designated locations would be modified to have two wells at each <br />location to monitor each zone, therefore, a total of 6 wells would be installed. TR -03 was submitted <br />to address the details of the well installations. This narrative was included in Appendix 15. Maps G- <br />14 and G -15 were added to the permit for this Revision. <br />TR -03 was approved in May of 2014. <br />In May /June of 2014, the mine operator changed from Star Mine Operations to Fortune Revenue <br />Silver Mines, Inc. <br />Revenue Mine January 2015 TR O5 i -5 <br />
