May 2014 inspection, OSMRE took issue with the excessive rills and gullies located on <br />the face of Gob Pile #4. At that time, the operator had finished final grading on the <br />western end and was beginning to spread the subsoil material prior to topsoil <br />redistribution. OSMRE's concern was based on a discussion with the operator who <br />stated that there were no plans to regrade these erosional features prior to subsoil and <br />topsoil redistribution. For this reason, OSMRE recommended better erosion BMP <br />implementation to prevent uncontrolled drainage and also encouraged the operator to <br />take a more proactive approach to preventative maintenance. Additionally, OSMRE <br />included language in the May 2014 inspection report that a failure to do so could prove <br />costly when determining reclamation success at bond release in the future. <br />OSMRE was informed by the operator during this inspection that the rills and gullies <br />were regraded prior to any subsoil /topsoil redistribution. Upon completion of topsoil <br />redistribution, vegetation matting was installed and appeared to be functioning as <br />designed. No issues or concerns were noted with regard to installation techniques. <br />Straw wattles have also been installed on top of the vegetation matting as additional <br />BMPs. <br />Additional confirmation of the aforementioned rill and gully repair was obtained via <br />conversation with Susan Burgmaier and a thorough review of previous inspection <br />reports since May 2014. <br />Roads (Rule 4.03): <br />All haul roads inspected appeared in good condition. NOTE: During the May 2014 <br />inspection, water from the wash out plant was observed draining across the road into an <br />area that was not designed to receive drainage; however, this issue is currently being <br />addressed pending approval of TR -91. Mine personnel were actively performing <br />maintenance on drainage ditches and culverts in this area at the time of inspection. <br />Signs and Markers (Rule 4.02): <br />The mine identification sign at the entrance from State Highway 133 is intact, clearly <br />visible and contained all necessary information in accordance with 4.02.2. Additionally, <br />the disturbed area perimeter and permit boundary areas have also been clearly marked <br />and are easily visible throughout in accordance with 4.02.3. The topsoil stockpile was <br />marked in accordance with 4.02.7. MSHA identification signs were also observed at <br />Gob Piles #1 and #2. Identification signage was observed at topsoil stockpile D. <br />Disposal of Noncoal Waste (Rule 4.11.4): <br />All areas inspected were clean and orderly. Specifically, those areas are as follows: <br />1. Fan Bench <br />2. Fuel Storage and Coal Crusher <br />3. "B" Bench Portal <br />4. Terror Creek Vent Shaft <br />5. Ponds B, C, D, J, F, and K <br />6. Loadout <br />2 <br />
