The following note shall be placed on the Special Review Permit <br /> plat prior to recording: <br /> NOTE: The Mined Land Reclamation Board has the authority <br /> to issue permits for mining and reclamation <br /> activities. Activities related to mining and <br /> reclamation are, therefore, under the jurisdiction <br /> of the Mined Land Reclamation Board; however, Wald <br /> County has jurisdiction of those matters outlined in <br /> the Development Standards. <br /> 4. Within 30 days of approval by the Board of County Commissioners: <br /> a. The applicant shall amend the existing road maintenance <br /> agreement with the Weld County Commissioners and the City <br /> of Greeley Public Works Department. The agreements shall <br /> mitigate impacts of truck traffic generated on Balsam <br /> Avenue by the stockpiling, recycling, and processing of <br /> demolition materials and the gravel mining operation. <br /> b. Any required emissions permit shall be obtained from the <br /> Air Pollution Control Division, Colorado Department of <br /> Health for fugitive dust and odors. <br /> c. Adequate toilet facilities served by an individual sewage <br /> disposal system, shall be installed. The toilet facilities <br /> shall be installed according to the Weld County Individual <br /> Sewage Disposal Regulations. <br /> d. Any required NPDES permit shall be obtained from the Water <br /> Quality Control Division of the Colorado Department of <br /> Health and/or amended, if appropriate for the proposed <br /> operations. <br /> e. A dust abatement plan shall be submitted to the Weld County <br /> Health Department for approval. The facility shall have <br /> sufficient equipment available to implement the dust <br /> control as required by the Health Department. This plan <br /> shall address the expected traffic volume, and sat <br /> standards for control. <br /> 5. The applicant shall submit evidence of compliance with Section <br /> 44.4.10 of the Weld County Zoning Ordinance regarding Insurance, <br /> to the Department of Planning Services prior to the start of the <br /> sand and gravel operation. <br /> 6. The applicant and/or operator of the sand and gravel operation <br /> shall notify the Department of Planning Services, in writing, <br /> when starting the sand and gravel operation on site. <br /> 920610 <br />
