The Applicant has identified the existing access road from Highway 93 as a permanent man- <br /> made structure. However,the owner of the structure was not identified and no structure <br /> agreement was provided. Please identify the owner of the access road and provide a structure <br /> agreement if the Operator is not the owner of this structure. <br /> 40. The maps provided as part of the Amendment submittal depict utility poles within the permit <br /> boundary. Please identify the owner of the utility and provide a structure agreement if the <br /> Operator is not the owner of this structure. <br /> 41. The maps provided as part of the Amendment submittal depict several buildings that are <br /> within 200 feet of the southwest corner of the permit boundary. Please identify the owner of <br /> the buildings and provide a structure agreement if the Operator is not the owner of the <br /> structures. <br /> 6.5 Geotechnical Stability Exhibit <br /> 42. The Division is currently reviewing the geotechnical analysis which was submitted to <br /> demonstrate the stability of the proposed reclaimed slopes. Comments regarding the stability <br /> analysis will be forthcoming. <br /> This concludes the Division's preliminary adequacy review of this application. Please remember <br /> that the decision date for this application is February 24, 2015. As previously mentioned, if you <br /> are unable to provide satisfactory responses to any inadequacies prior to this date, it will be your <br /> responsibility to request an extension of time to allow for continued review of this <br /> application.
