Please see attached map <br /> 12.The Mining Plan Map shows there is a parcel of private property on the east side of the <br /> proposed mine site. Please identify the owner of record of the surface on the Mining Plan Map. <br /> The owner of the private property is a patented mining claim which would be potentially <br /> returned to National Forest use as well. <br /> 13.As required by Rule 6.3.5(2)(f), all existing disturbance within and/or adjacent to the permit <br /> boundary must be outlined and labeled on the Mining Plan Map. Re-disturbance of previously <br /> disturbed areas, by the proposed mining operation, must be included in the permit area and <br /> addressed in Exhibit D—Reclamation Plan. Please see labels on attached map <br /> 14.The Mining Plan Map states the west half of the Rise Above claim (CMC 285712)corresponds <br /> to the permit boundary. The Rise Above claim is 20.66 acres in size, which makes half of the <br /> claim is 10.33 acres. The Applicant has specified the permitted area is 9.9 acres. Please <br /> remove the statement from the Mining Plan Map and ensure the permit boundary delineates <br /> an area of exactly 9.9 acres. This change has been made,please see attached map. <br /> 15.The Reclamation Plan Map must be revised to include the following elements: <br /> a) The name of the Applicant. Names of applicants are at the bottom left corner of attached <br /> map <br /> b) The map must be signed by the person who prepared it.Signatures of applicants are at the <br /> top right corner of attached map. <br /> c) Give the date prepared. 12/17/2014 as is written on the top left corner of attached map. <br /> d) Label the permit boundary.Permit boundary is labeled on attached map. <br /> e) Include a north arrow. North arrow is specified on attached map. <br /> f) The map must include a scale. Alternately, the Applicant can specify the length of each <br /> side of the permit boundary.Scale is included on attached map. <br /> g) State the average thickness of replaced overburden; you may make a note on the map that <br /> states how much overburden will be replaced. Approximately 10x8x2 cubic feet of <br /> overburden per hole will be replaced. <br /> h) State the average thickness of replaced topsoil. Soil replaced will be approximately 3 to 5 <br /> inches deep according to surrounding soils. <br /> i) Show the gradient of all reclaimed slopes (horizontal:vertical) sufficient to describe the <br /> post-mine topography. Gradient slopes will be less than 3 percent on active, permitted area. <br /> 6.3.6 Exhibit F —List of Other Permits and Licenses Required <br /> 16. Please indicate if you have contacted the Colorado Department of Public Health and <br /> Environment —Water Quality Control Division in regard to the need for a storm water <br /> discharge permit. If you have not already done so, contact the Water Quality Control <br />
