PERMIT#:M-1991-080 <br /> INSPECTOR'S INITIALS:TC1 <br /> INSPECTION DATE:January 28,2014 <br /> OBSERVATIONS <br /> This inspection was conducted as part of the regular monitoring program. The Operator (Russ Van Egmond) <br /> was represented by Mr. Jacob Van Egmond who was present for the inspection. This is a 110c gravel mine. <br /> The mine was not operating at the time of the inspection. <br /> The Stones Ranch Gravel Pit entrance is located approximately 5.5 miles northwest of Cripple Creek, off <br /> County Road 1. <br /> Inspection: <br /> Approximately two inches of snow covered the site. A permit sign was displayed at the site entrance (see <br /> Photo 1). The permit boundary appeared to be fenced along the entire perimeter (see Photo 2). Portable <br /> conveyors and crushers (see Photo 3) were observed on site as well as two excavators (see Photo 4), semi <br /> trailer (see Photo 5), and grizzly screen (see Photo 6). A truck scale was observed near the fenced off A-frame <br /> (see Photo 7) at the pit entrance. Mr. Van Egmond stated the A-frame was used as an office and that is was <br /> outside the permit area. <br /> Mr. Van Egmond explained that blasting operations are contracted out and performed two to four times per <br /> year. <br /> Due to the snow cover, this specialist was unable to determine whether the site was experiencing any <br /> problems with erosion/sedimentation or noxious weeds. <br /> Records: Note the Division has incomplete records for this permit. As such, a problem is cited on page 1 of this <br /> report related to the availability of records. <br /> 1) The Division does not have the following original permit exhibits on file: <br /> a) Exhibit E — Mine and Reclamation Plan Maps (Reference Rule 6.3.5). The original application <br /> (enclosed) references as many as 6 maps. <br /> i) The Division's database indicates total permitted area is 10 acres. There may be a maximum <br /> affected area limit of 8 acres, but that is undocumented <br /> 2) The Division has incomplete records or documentation for the following permit exhibits: <br /> a) Exhibit C—Mining Plan (Reference Rule 6.3.3). Specifically: <br /> i) Rule 6.36.3(k) — discussion on "if refuse and acid or toxic producing materials are exposed during <br /> mining"; <br /> ii) Rule 6.3.3(1) — "describe what measures will be taken to minimize disturbance to the hydrologic <br /> balance, prevent off-site damage, and provide for a stable configuration of the reclaimed area <br /> consistent with the proposed future land use."; and <br /> iii) 6.3.3(p) — "Specify if explosives will be used in conjunction with the mining or reclamation." This <br /> item needs specific clarification as Mr. Van Egmond stated during the inspection that the Operator <br /> contracts out their blasting operations. A statement that blasting is used on site and a submittal of <br /> the blasting plan used by the blasting contractor may satisfy this requirement. <br /> b) Exhibit D— Reclamation Plan (Reference Rule 6.3.4). Specifically: <br /> i) Rule 6.3.4(2) — No copy of the original estimated costs for reclamation is on file with the Division. <br /> However, due to the significant bond estimate reduction associated with the approval of TR-01, a <br /> new cost estimate is required. <br /> Page 2 of 8 <br />
