Compliance With Mine Plan N - Support Facilities Not On -site <br />N - Other N - Special Categories Of Mining <br />N - Processing Waste N - Topsoil <br />COMMENTS <br />This was a complete inspection of the North Thompson Creek Mine conducted by Tabetha Lynch and Brock <br />Bowles of the Colorado Division of Reclamation, Mining and Safety. The weather conditions were partly cloudy <br />and about 40 degrees Farenheit. The ground on the mine site was snow covered on the south side of the mine area. <br />The road was passable with a few muddy areas. <br />AVAILABILITY OF RECORDS — Rule 5.02.4(1): <br />The mine records were available at the Garfield County Courthouse in Glenwood Springs. Several <br />records need to be updated. The insurance certificate expired in September 2014, and the most up to date <br />insurance certificate needs to be included. The last DMR available was February 2014, and all records up to date <br />need to be included. The last quarterly pond inspection included was the second quarter 2014, and quarterly <br />inspection need to be updated. As a note to the operator the NPDES Permit will expire January 31st 2015. See the <br />availability of records form at the end of this inspection report for more detailed information. <br />BACKFILL and GRADING — Rule 4.14 <br />Contemporaneous Reclamation 4.14.1; Approximate Original Contour 4.14.2; Highwall Elimination 4.14.1(2)(f); <br />Steep Slopes 4.14.2, 4.27; Handling of Acid and Toxic Materials 4.14.3; Stabilization of Rills and Gullies 4.14.6: <br />The area around portal #1 appeared stable. There was no evidence of slumping or cracking. <br />HYDROLOGIC BALANCE - Rule 4.05 <br />Drainage Control 4.05.1, 4.05.2, 4.05.3; Siltation Structures 4.05.5, 4.05.6; Discharge Structures 4.05.7, 4.05.10; <br />Diversions 4.05.4; Effluent Limits 4.05.2; Ground Water Monitoring 4.05.13; Surface Water Monitoring 4.05.13; <br />Drainage — Acid and Toxic Materials 4.05.8; Impoundments 4.05.6, 4.05.9; Stream Buffer Zones 4.05.18: <br />Both portal # 1 and 3 were discharging to the long pond. Discharge from the air shaft access culvert was <br />noted, but no iron staining was observed. This water was discharging into North Thompson Creek at a field <br />observed volume of approximately half a gallon a minute. Mine portal # 3 manhole cover was checked with no <br />observed problems or changes.
