Compliance With Mine Plan Y - Support Facilities Not On-site <br /> NA - Other NA - Special Categories Of Mining <br /> Y -Processing Waste R -Topsoil <br /> COMMENTS <br /> This was a complete inspection of the New Horizon North Mine conducted on September 18, 2014 by Mike <br /> Boulay of the Division of Reclamation,Mining and Safety. Frank Ferris of Western Fuels-Colorado (WFC)was <br /> present during the entire inspection. Weather conditions were warm and clear and ground conditions were dry. <br /> WFC was hauling coal from the mine to the power plant at the time of the inspection. Drilling in preparation for <br /> blasting the Bench 2 overburden was in progress and overburden material was being hauled from the west side of <br /> the pit for backfilling and grading operations. <br /> AVAILABILITY OF RECORDS—Rule 5.02.4(1): <br /> The availability of records was checked on the morning of 9/18 prior to the ground inspection. All required <br /> records were on file and available for review at the mine office, with exception of the Insurance Certificate. Please <br /> update the records file with a current Insurance Certificate. No other problems were noted with the records <br /> review. Please see the attached Availability of Records checklist at the end of this inspection report. The AVS <br /> Annual Review Inspection was also completed on this date. On site observations were made and direct inquiry of <br /> site personnel were conducted to verify information on the Mine ID sign,the identity of the permittee and <br /> operator, and the MSHA ID number. <br /> HYDROLOGIC BALANCE -Rule 4.05 <br /> Drainage Control 4.05.1,4.05.2,4.05.3; Siltation Structures 4.05.5,4.05.6; Discharge Structures 4.05.7,4.05.10; <br /> Diversions 4.05.4;Effluent Limits 4.05.2; Ground Water Monitoring 4.05.13; Surface Water Monitoring 4.05.13; <br /> Drainage—Acid and Toxic Materials 4.05.8; Impoundments 4.05.6,4.05.9; Stream Buffer Zones 4.05.18: <br /> There is an overall increase in groundwater flowing into the pit from the lower end of the highwall and from the <br /> pit bottom(see Photograph No. 1). WFC is pumping groundwater out of the pit to Pond NHN-001. <br /> Sedimentation Pond NHN-00I was inspected and there is a significant amount of water contained in the pond,but <br /> well below the level of the primary spillway. As noted during the June, 2014 inspection, some rills were forming <br /> on the side slopes of the pond. These have been repaired(see Photograph No. 2).
