October 28, 2014 C- 1981 - 022 /Elk Creek Mine JRS
some vegetation but could probably benefit from being reseeded. There was some minor erosion at the base of the
topsoil pile that will need to be monitored and repaired as necessary. There was a sign in place on the topsoil
stockpile. There were no offsite impacts noted at this GVB.
- GVB 19 -02 was drilled and temporarily capped but did not appear to have the annulus grouted to the surface.
The drill pad was stable and had a partial berm around it. There appeared to be positive drainage off of the pad.
There were straw wattles in the drainage ditch and several along the outslope of the pad. The outslopes of the pad
had good vegetative cover. The topsoil stockpile had some vegetation but could probably benefit from being
reseeded. There was a sign in place on the topsoil stockpile. There were no offsite impacts noted at this GVB.
- GVB 19 -03B was drilled and temporarily capped and appeared to have the annulus grouted to the surface. The
drill pad was stable and had a partial berm around it. There appeared to be positive drainage off of the pad. There
were straw wattles in the drainage ditch and several along the outslope of the pad. The outslopes of the pad had
good vegetative cover. The topsoil stockpile had good vegetation on it. There was a sign in place on the topsoil
stockpile. There was a small sediment plume at the base of the pile below the ditch that extended several feet but
it was determined that it was not a problem. Overall, there were no offsite impacts noted at this GVB.
- GVB 19 -04 was drilled and temporarily capped and appeared to have the annulus grouted to the surface. The
drill pad was stable and had a partial berm around it. There appeared to be positive drainage off of the pad. There
were straw wattles in the drainage ditch and several along the outslope of the pad. The outslopes of the pad had
good vegetative cover. The topsoil stockpile had good vegetation on it. There was a sign in place on the topsoil
stockpile. There were no offsite impacts noted at this GVB.
- GVB 19 -04B was drilled and temporarily capped and appeared to have the annulus grouted to the surface. The
drill pad was stable and had a partial berm around it.
- GVB 19 -05 was drilled and temporarily capped and but the annulus did not appear to be grouted to the surface.
The drill pad was stable and had a partial berm around it. There appeared to be positive drainage off of the pad.
There were no straw wattles in the drainage ditch. Straw wattles will need to be placed in this ditch for sediment
control. The outslopes of the pad was well vegetated and had wattles along the base of the pad. The topsoil
stockpile had good vegetation on it. There was a sign in place on the topsoil stockpile. There were no offsite
impacts noted at this GVB.
- GVB 19 -08 was drilled and temporarily capped and appeared to have the annulus grouted to the surface. The
drill pad was stable and had a partial berm around it. There appeared to be positive drainage off of the pad. There
were no straw wattles in the drainage ditch and none along the outslope of the pad. Straw wattles will need to be
placed in the ditch and at the outslopes of this pad. The topsoil stockpile was a long pile that is basically a large
windrow. The pile had decent vegetation on it. There was a sign in place on the topsoil stockpile but, given the
long, thin pile, it could benefit from a second sign at the other end. There also appeared to be a small strip of
topsoil that was not salvaged. The Division estimated that this small strip was around 10 yards. There were no
offsite impacts noted at this GVB.
- GVB 19 -09 was drilled and appeared to have the annulus grouted to the surface. This GVB did not have a pump
attached and was not temporarily capped. The drill pad was stable and had a partial berm around it. There
appeared to be positive drainage off of the pad. There were straw wattles in the drainage ditch and along the
outslope of the pad. The outslopes of the pad had good vegetative cover. The topsoil stockpile was fairly long
Number of Partial Inspection this Fiscal Year: 3
Number of Complete Inspections this Fiscal Year: 1
Page 3 of 34
