November 4, 2014 C- 2010- 089/New Horizon North Mine BFB <br />SIGNS AND MARKERS — Rule 4.02: <br />The stream buffer zone signs were in place around Meehan Draw and the northwestern corner of the <br />permit boundary (photo 5). <br />Mine identification signs need to be installed at all mine entrances. Several signs were missing at entrances to the <br />permit area from public roads. A locked gate is still considered an entrance and needs a mine identification sign as <br />per Rule 4.02.2(1). A mine entrance sign is also needed at the entrance to the Pond 1 primary spillway area, south <br />of AA road. <br />Permit boundary markers are needed on the south permit boundary, along the south equipment corridor and along <br />the access road to the NH Pond. <br />TOPSOIL — Rule 4.06 <br />Removal 4.06.2; Substitute Materials 4.06.4(4); Storage and Protection 4.06.3; Redistribution 4.06.4: <br />The topsoil that had been spread between the facilities area and the SW -TS topsoil piles appears to have <br />been ripped (photo 6).
