October 15, 2014C-1996-083/Bowie No. 2 MineSLB <br />A black plastic culvert not identified on permit application maps was in place on the southeast side of Topsoil <br />. The culvert should <br />Stockpile F. The culvert was used by the former landowner to cross an irrigation ditch <br />either be removed or identified on permit application maps. <br />If BRL elects to identify the culvert on the permit <br />application maps, this could be accomplished through TR-91, currently under review. <br />Gob Pile #3 Coverfill Stockpile <br />The berm around the south cover fill stockpile at Gob Pile #3 has been constructed so that it will impound water. <br />An opening should be cut in the berm to ensure that runoff from the stockpile will drain to culvert J1 <br />. This <br />issue was identified on October 16, 2014; BRL reported on October 17, 2014 that the problem would be addressed <br />that day. The Division did not verify that it had been resolved. <br />Ponds <br />Ponds B, C, D, F, and J held water; Pond K was damp. None of the ponds were being discharged off-site at the <br />time of the inspection. Pond F was being pumped to Pond C, and Pond C was being pumped to the wash plant. <br />BRL had set up containment structures for fuel containers at each of the sediment ponds, to prevent spills from <br />entering the ponds. <br />The primary discharge pipe at Pond B, located on the south face of the embankment, is piped directly to the Deer <br />Trail Ditch. The Deer Trail Ditch flows in a pipe along the embankment of Pond B, across the Fire Mountain <br />Canal, through a Parshall Flume west of the Terror Creek Loadout. During the inspection, it became apparent that <br />BRL might besampling the quality of Pond B discharge at the discharge pipe on the embankment, but measuring <br />flow at the Parshall flume. Flow at the Parshall flume, depending on whether the Deer Trail Ditch is running, <br />Effective immediately, BRL should measure <br />would consist of Pond B discharge and Deer Trail Ditch flow. <br />flow and collect water quality samples at the outfall of Pond B before flow is comingled with water flowing <br />in the Deer Trail Ditch. <br />Surface Water Observations <br />The pond at Pond and Spring 19, recentlyundermined, was holding waterflowing from the spring. <br />The West Fork of Terror Creek exhibited low flow conditions; flow at both flume locations (SW-1 and <br />SW-12) was entirely within the newly constructed concrete flumes. The flow level at the upper flume was <br />about 3” below the top of the flume (observed 10/15/2014 a.m.); the lower flume level was approximately <br />4-5” below the top of the flume (observed 10/15/2014 p.m.) <br />Hubbard Creek and the East Fork of Terror Creek were both flowing. <br />Flow in the North Fork of the Gunnison was low and turbid. <br />OTHER (SPECIFY): <br />Permitting Defect –Permit and Disturbed Area Boundary <br />The yellow sheds and material storage area yard just west of the conveyor and north of the canal are located <br />outside of the permit boundary designated on permit application maps.
