As previously discussed, the major elements of the project were designed to address these <br /> objectives. <br /> Project requirements for construction quality control and construction quality assurance are <br /> defined in the Golden Gilpin Mill Site Mine Reclamation Construction Documents (WRC 2005). <br /> Each section of the project specifications contained information or requirements as appropriate <br /> for standard references, submittals, quality assurance, installation and testing. <br /> The construction contract outlined a number of quality assurance requirements to be met by the <br /> contractor during project construction. The primary document governing construction Quality <br /> Assurance/Quality Control (QA/QC) was a QA/QC Plan submitted by the contractor and <br /> approved by the engineer. The QA/QC plan outlined organizational and schedule control,record <br /> keeping and reporting requirements, and submittals and testing requirements. In addition to <br /> approving the QA/QC Plan, the engineer was also responsible for reviewing and approving all <br /> material submittals and test results to ensure compliance with project specifications. <br /> CDPHE staff performed periodic construction oversight of the project,with support from the <br /> project engineer. Additional quality assurance was provided through geotechnical and materials <br /> testing performed as specified in the construction documents. Geocal Inc., a local consultant, <br /> was retained by CDPHE to perform all geotechnical and materials testing required by the <br /> project. The project engineer reviewed and approved all test results. The engineer was also on- <br /> site as needed for progress meetings and during select times during construction to ensure that all <br /> work was performed in compliance with the construction drawings and specifications. <br /> VI. FINAL INSPECTION AND CERTIFICATION <br /> CDPHE developed an initial punch list for project completion on July 29, 2008. On that date, a <br /> pre-final inspection of the clean up was conducted by Doug Jamison of CDPHE with Nathan <br /> Torrey of WRC and Josh Romans of LHE in attendance. The final inspection was conducted on <br /> September 19, 2008. Attendance at the final inspection consisted of the Doug Jamison of <br /> CDPHE and Scott Aschenbrenner of LHE. On September 23, 2008, a final site walkthrough was <br /> conducted by CDPHE and WRC Engineering Inc. <br /> The pre-final and final inspections consisted of reviewing each specific construction activity <br /> outlined in the contract documents. Deficiencies were noted on the inspection checklist and the <br /> contractor was notified verbally at the time of inspection and in writing. All of the deficiencies <br /> noted during the pre-final inspection had been corrected at the time of the final inspection. At <br /> the time of the final inspection, all punch list items identified during the pre-final inspection, <br /> except for final demobilization, had been completed at the time of the final inspection. Final <br /> demobilization had been completed when CDPHE and WRC conducted a final site walkthrough <br /> on September 23, 2008. At the date of this report, the only work remaining under the <br /> construction contract between CDPHE and LHE is completion and submittal of as-built <br /> construction drawings and any warranty work that might be necessary. A warranty inspection <br /> will be conducted in August of 2009. <br />
