TERMS & CONDITIONS
1. This permit is issued in coniunction with Permits Jis 399055 and 7fe ?99057. Submittal for design shall include
considerations for all of these permits. (only one submittal required) If there are any questions re~ardins this

'_. The Pernti«ee shall desi~en and submit for approval a left turn deceleration and acceleration lane 16 feet in width in
conformance with section ~ of the State Highway Access Code, 2CCR 60I
3. The Permittee shall design and submit for approval a right turn deceleration and acceleration lane 12 fee[ in width
in conformance with section 4 of the State Highway Access Code, 2CCR 601-I.
4. Design of this access with auxiliary lanes shall include, a[ a minimum. I'h-inch overla}•.
5. A Notice to Proceed. CDOT Form 1165 is required before besinning the construction of the access or any activity
within the highway right-of-H•ay. All submittals. documents, plans and other items that must be completed, shall
be submitted~and approved before a Notice to Proceed will be issued. Contact the Department (or local issuing
authority) for the Notice to Proceed.
6. The following items aze required before a Notice to Proceed will be issued:
(a) Construction Plans Stamped by a Colorado Registered Professional Engineer.
(b) Certificate of Insurance Liability as per Section 2.3(I 1)(h) of the Access Code.
(c) Traffic Control Plan
7. The Permittee shall refer to all additional s[andazd requirements on the back of this permit and any enclosed
additional terms. conditions. exhibits and noted attachments.
8. This Permit is issued in accordance with the State Highway Access Code (2 CCR b01-1), and is based in pan upon
the information submitted by the Permittee. This permit is only for [he use and purpose stated in the Application
and Permit. Any changes in vaffic volumes or type, drainage, or other operational aspects may render this permit
void, requiring a new permit to be applied for based upon existing and anticipated future conditions.
9. Upon completion of the access, the applicant shall notify [he Access Manager by certified mail within 10 days a[:

10. Nothing in this permit shall prohibit the chief engineer from exercising the right granted in CRS 43-3-102
Including but not limited [o restricting left hand toms by construction of physical medial sepazations.
11. All costs associated with the installation of traffic ligha for this access are the responsibility of [he Permittee. This
includes but is not limited to the design, consruction, utility relocation, testing of materials and inspection.
11. ll%a[er, sanitary, sewer, gas. elecu-ical, communication, landscaping, and telephone installations will require
individual additional permits.
13. The Permittee is responsible for obtaining any necessary additional federal, stale and~'or City/County permits or
clearances required for construction of the access. Approval of [his access permit does not constitute verification
of this action by the Permittee.
14. Any work within State Highway right-of-way shall begin after 8:30 A.M. and all work and equipment shall be off
the highway BEFORE 3:30 P.M. each day.
15. No highway lane closures or one-way traffic will be allowed.
16. No work will be allowed at night, Saturda}•s. Sundays and legal holidays without prior authorization from the •
Department. The Department may also resvict work within the State Highway right-ot-way during adverse
weather conditions.
