Mr. Peter Hays, DBMS
May 14,2014
Phone: 720-942-3518
timothy haynes@fmi.com
(d) No monitoring or analyses were performed for this situation since Mill water was not
released off-site and did not impact any natural water body.
(e) While the exact cause of this upset has yet to be determined, investigations conducted
indicate that the overall start and stop of flows between the emergency cutout and the
TDL while refurbishment construction is being completed in this area is likely the source
of the issue. The following day, flow was returned to the TDL and issues similar to those
mentioned in this follow-up notification did not occur, nor was there any indication that
they might. The tailing engineering department, along with tailing operations personnel
will continue to monitor this closely to ensure the issue does not repeat itself.
If you have any additional questions or concerns, please feel free to contact me at 720-942-3518 or Bryce
Romig at 720-942-3231.
Sincerely,
4-L'
Tim Haynes
Sr. Environmental Engineer
Climax Molybdenum Company
Henderson Mill
CC (via email):
B.
