Climax Molybdenum COLORADO Oe de sonOMS
A Freeport-McMoRan Company 19302 County Road 3
Parshall, CO 80468
Phone (303)569-3221
October 14, 2014 RECEIVED
Via Email and UPS Priority Mail OCT
Mr. Peter Hays ✓VISION OFRECLAMATION
Division of Reclamation, Mining and Safety MININGANDSAFETy
1313 Sherman St., Rm. 215
Denver, CO 80203
Re: Follow-up Notice for Mill Tailing Delivery Line Leak, Permit No. M-1977-342
Mr. Hays:
Henderson Mill is providing this written follow-up notice to a phone call made to you on Oct 9, 2014
pursuant to Rule 8.1 and 8.2 of the DBMS Rules and Regulations.
This notice is regarding tailing slurry which escaped from a joint in the #12 droptower, along with a larger
flow of material that flowed from the #5 droptower during the same event. The Tailing Delivery Line
(hereafter referred to as the TDL) is designated as an environmental protection facility in Henderson's
Environmental Protection Plan. The leak is believed to have begun at 8:20pm the evening of October 8,
2014. It is unclear how much tailing was released, but general milling capabilities would put the worst
case scenario at 8,000 gallons per minute of slurry through a duration period from 8:20pm to 9:00pm.
Given the remaining material on the ground, and the overall distance the material traveled, it does not
appear that this magnitude of tailing was released onto the TDL right-of-way. To that end, it is our
understanding that there was continued flow through the TDL, while the release was occurring. All slurry
released from the line flowed along the alignment of the tailing delivery line to the No. 1 Cutout structure,
below the #12 droptower, where it flowed through a culvert and comingled with process waters
impounded in its intended location.
(a) After discovering the issue, Henderson employees immediately stopped flow to the TDL
and diverted it to the Emergency Cutout Line which stopped the flow out of the leaking
line.
(b) Mill tailing slurry flow did not adversely impact human health, property or the
environment. Some slurry accumulated in place, but the majority flowed along the TDL
alignment and access road. Any drainage from this area flows underneath County Road
3 and along a private Mill access road to a culvert, where it comingled with process
waters flowing into the tailing impoundment.
(c) The contact person for this event is:
Tim Haynes
19302 County Road 3,
Parshall, CO 80468
