2) The Division's staff hydrologist is currently reviewing the revisions made to Section 2.05.6 of the <br />permit, specifically the section titled "Postmining or Interim Discharge — No. 5A and No. 6 <br />Mines ". If additional adequacy items are found, they will be forwarded to MCM as soon as <br />possible. <br />a. DRMS Response: After review by the Division's staff hydrologist, no additional <br />adequacy items were identified for this item. This item is considered resolved. <br />4.05.13 — Surface and Ground Water Monitoring: <br />1) Revised Map 11 does not depict stream water quality monitoring site Y -I or the North Spring <br />(Lippard Spring No. 1). Please add these features to this map and submit a revised copy. <br />a. MCM Response: The referenced map has been revised and is included with the <br />responses. <br />b. DRMS Response: The North Spring on the revised Map #11 is in a different place than <br />depicted on the currently approved Map 11. Please explain this discrepancy. <br />Mid -Term #6; Adequacy Review Issues <br />3. Permit page 2.05 -7 contains a discussion of the drilling activities and planned future monitoring of wells <br />94 -001 and 95 -14. Based on a review of current drill holes and hydrological monitoring sites, the status of <br />these wells is unclear. Hole 95 -14 is shown on Map 11 — Hydrologic Monitoring Program Location Map <br />but it is not sampled, included in the Annual Hydrology Report or shown on AHR Figure 2 — Hydrologic <br />Monitoring Program Location Map. The Division could find no other reference to hole 94 -001. Please <br />review the information on page 2.05 -7 for holes 94 -001 and 95 -14 and update their status as necessary. <br />Please provide the Division with a discussion of proposed well 95 -14 and its possible inclusion in the <br />hydrologic monitoring plan. <br />a. MCM Response: The specific Permit text discussion relating to the referenced wells has been <br />reviewed and revised to clarify and update the status and use of these wells. Copies of the revised <br />Permit discussion accompany these responses for replacement in the PAP. <br />b. DRMS Response: The information regarding this well was revised and placed on page 2.05 -2. <br />The revised page indicated Well DPP -94 -001 was plugged and sealed and released by CDRMS in <br />2012. A record of the abandonment of this well could not be located. Please provide the <br />abandonment report for this well. <br />c. MCM Response #2: Further review of the status of Well DPP -94 -001 indicates that it was not <br />abandoned and that it still exists. The relevant text has been revised to reflect this correction and <br />the revised page is included with these responses. MCM has identified several old wells that <br />remain from previous owners /operations, and is currently developing plans to plug and seal these <br />wells. <br />d. DRMS Response #2: The response indicates that this well still exists and that the relevant text has <br />been revised to reflect this correction. However, revised page 2.05 -2.1 now includes the discussion <br />of well DPP -94 -001 and it indicates this well has been plugged and sealed as well as release from <br />bond by the CDRMS. Please correct this statement on the revised page to reflect the actual <br />condition of the well. <br />
