COLORADO Division of Reclamation, A7Mining and Safety Department of Natural Resources 1313 Sherman Street, Room 215 Denver, CO 80203 September 11, 2014 Mr. Russell Painter 1761 Pikes Peak Dr. Florissant, CO 80816 RE: Amazonite #VIII, File No. M- 2014 -042; Preliminary Adequacy Review Dear Mr. Painter: The Division of Reclamation, Mining and Safety (Division) has completed its preliminary adequacy review of your 11 0(l) Hard Rock Limited Impact Operation reclamation permit application. The application was received on August 1, 2014 and called complete for review on August 15, 2014. The decision date for this application is September 15, 2014. Please be advised that if you are unable to satisfactorily address any concerns identified in this review before the decision date, it will be your responsibility to request an extension of the review period. If there are outstanding issues that have not been adequately addressed prior to the end of the review period, and no extension has been requested, the Division will deny this application. The review consisted of comparing the application content with specific requirements of Rules 1.6.2, 3, 6. 1, 6.2, 6.3 and 6.5 of the Minerals Rules and Regulations of the Colorado Mined Land Reclamation Board for Hard Rock, Metal, and Designated Mining Operations (Rule). Any inadequacies are identified under the respective exhibit heading along with suggested actions to correct them. 1.6.2 GENERAL APPLICANT PROCEDURES The Applicant is required to publish a public notice in a newspaper of general circulation in the locality of the proposed mining operation within ten (10) days after the Office notifies the Applicant that the application is considered filed. Please provide the Division with proof of publication and the date on which it was published. 6.3 SPECIFIC EXHIBIT REQUIREMENTS — 110(1) LIMITED IMPACT OPERATIONS The following items must be addressed by the applicant in order to satisfy the requirements of C.R.S. 34- 32 -101 et seq. and the Mineral Rules and Regulations of the Mined Land Reclamation Board: 6.3.4 EXHIBIT D — Reclamation Plan 2. Reclamation Cost Estimate — The Division has a Memorandum of Understanding with the USFS requiring acceptance of the Division's estimated reclamation bond. Attached are the preliminary cost estimate worksheets for the financial warranty. Please review them as this is what the Division will require for a bond unless the USFS requests adjustments.
