PERMIT #: M- 2001 -085 <br />INSPECTOR'S INITIALS: TOD <br />INSPECTION DATE: August 15, 2014 <br />INSPECTION TOPIC: Gen. Compliance With Mine Plan <br />PROBLEM: The Operator is extracting material that was committed to uses in final reclamation and by doing <br />so is conducting mining operations. <br />CORRECTIVE ACTIONS: The Operator must either: <br />1. Cease extraction and exportation of material generated from within the pen-nit boundary <br />e <br />2. The Operator shall submit a Technical Revision, with the required $216 revision fee, to update and clarify the <br />current mine plan and the reclamation cost estimate, to reflect existing and proposed activities by the corrective <br />action date. The Technical Revision shall include the following: <br />a. A Revised 6.4.4 Exhibit D - Mining Plan, <br />b. A Demonstration of completed reclamation ( cubic yards backfilled), <br />c. A Demonstration that identifies the remaining reclamation liability, and <br />d. A Revised Exhibit L - Reclamation Costs. <br />CORRECTIVE ACTION DUE DATE: 9/25/14 <br />Page 2 of 5 <br />
