A well permit has been obtained for the current use and exposed pond surface area of the <br />gravel pit in accordance with §37 -90- 137(2) and (11), C.R.S., permit no. 62970 -F. <br />3. Well permit no. 66319 -F was approved pursuant to §37 -90- 137(2), C.R.S., for the proposed <br />operational uses. <br />4. Approval of this SWSP is for the purposes stated herein and pumping is limited to the wells <br />identified above. Additional wells and /or additional uses for the water that is the subject of <br />this SWSP will be allowed only if a new SWSP is approved for those additional wells /uses <br />and such additional wells /uses. <br />5. The total surface area of the ground water exposed must not exceed 2.0 acres, which results in <br />a maximum evaporative annual loss at the Heit Sand and Gravel Mine of 5.82 acre -feet. <br />6. The annual amounts of water used at the Heit Sand and Gravel Mine shall not exceed 1.0 acre - <br />feet for dust control, and the total amount of aggregate washed shall not exceed 485,000 tons <br />per year, resulting in a consumptive use of 10.7 acre -feet of water. The annual amount of water <br />used at the site for operational purposes therefore shall not exceed 11.7 acre -feet. <br />7. Total consumption at the Heit Sand and Gravel Mine must not exceed the aforementioned <br />amounts unless an amendment is made to this plan. <br />8. Approval of this plan is for the purposes as stated herein. This office must first approve any <br />additional uses for the water. <br />9. All releases of replacement water must be sufficient to cover all out -of- priority depletions in <br />time, place, and amount and must be made under the direction and /or the approval of the <br />water commissioner. The release of replacement water may be aggregated to maximize <br />beneficial use. The water commissioner and /or the division engineer shall determine the rate <br />and timing of an aggregated release. <br />10. The replacement water that is the subject of this plan cannot be sold or leased to any other <br />entity. As a condition of subsequent renewals of this substitute water supply plan, the <br />replacement water must be appurtenant to this site until a plan for augmentation is obtained. <br />All replacement water must be concurrent with depletions in quantity, timing, and locations. <br />11. All diversions shall be measured in a manner acceptable to the Division Engineer. The <br />Applicant shall install and maintain such measuring devices as required by the Division <br />Engineer for operation of this SWSP. In addition, the Applicant shall maintain daily records <br />of all diversions, replacements, and the amount of water used for each particular purpose. <br />The Applicant shall provide a report of these records to the Division Engineer <br />( DivlAccountingta'�state.co.us), the South Platte River operations coordinator <br />(Brent.Schantz @state.co.us) and the water commissioner (Bill Schneider at <br />William. SchneiderCaa)state.co.us) on a monthly basis. The accounting must be submitted <br />within 30 calendar days of the end of the month for which the accounting is being made and <br />conform to the Administration Protocol "Augmentation Plan Accounting, Division One — <br />South Platte River' (attached). <br />In addition, the Applicant shall verify that the entity making replacements has included this <br />use on their accounting submitted to our office; for this plan, that entity is the City and County <br />of Broomfield (WDID 0202513). The report shall include an accounting of all fully <br />consumable effluent from Broomfield's wastewater treatment plant controlled by Broomfield, <br />showing the total volume of water under its control and the amount committed to each of the <br />recipients of the water. <br />12. The name, mailing address, and phone number of the contact person who will be <br />responsible for operation and accounting of this plan must be provided on the accounting <br />forms to the division engineer and water commissioner. <br />
