Compliance With Mine Plan <br />N - Support Facilities Not On -site <br />N - Other <br />N - Special Categories Of Mining <br />N - Processing Waste <br />N - Topsoil <br />COMMENTS <br />This was a complete inspection of the North Thompson Creek Mine conducted by Brock Bowles of the Colorado <br />Division Reclamation, Mining and Safety. The weather conditions were clear and warm. The ground was dry <br />except for a couple of puddles in the access road. The vegetation is still very green at the mine site and <br />surrounding areas. <br />AVAILABILITY OF RECORDS — Rule 5.02.4(1): <br />The mine records were available at the Garfield County Courthouse in Glenwood Springs. All necessary <br />records were available and up -to -date. See Availability of Records checklist at the end of this inspection report. <br />As a note to the operator: the insurance certificate expires September 2014 and the NPDES permit expires January <br />2015. <br />BACKFILL and GRADING — Rule 4.14 <br />Contemporaneous Reclamation 4.14.1; Approximate Original Contour 4.14.2; Highwall Elimination 4.14.1(2)(f); <br />Steep Slopes 4.14.2, 4.27; Handling of Acid and Toxic Materials 4.14.3; Stabilization of Rills and Gullies 4.14.6: <br />The 41 portal area was stable. There were no signs of slumping or cracking. <br />HYDROLOGIC BALANCE - Rule 4.05 <br />Drainage Control 4.05.1, 4.05.2, 4.053; Siltation Structures 4.05.5, 4.05.6; Discharge Structures 4.05.7, 4.05.10; <br />Diversions 4.05.4; Effluent Limits 4.05.2; Ground Water Monitoring 4.05.13; Surface Water Monitoring 4.05.13; <br />Drainage — Acid and Toxic Materials 4.05.8; Impoundments 4.05.6, 4.05.9; Stream Buffer Zones 4.05.18: <br />The discharge pipe from portal 41 crossing the creek is in good condition (photo 1). It is suspended from <br />wires on the east side of the bridge. Mines No. 1 and 3 were discharging water to the long pond (photo 2). <br />The long pond contained water and the bank was well vegetated and appeared to be stable. Ponds T1 and T2 <br />contained water and the banks appeared to be stable. The discharge from T2 was directed to Pond P9. P9 was not <br />discharging (photo 3). The refuse pile pond did not contain water.
