Please address the <br /> comments noted in the letters and make any changes to the application as necessary. <br /> 6.4 SPECIFIC EXHIBIT REQUIREMENTS— REGULAR 112 OPERATIONS <br /> The following items must be addressed by the Applicant in order to satisfy the requirements of <br /> C.R.S. 34-32.5-101 et seq. and the Mineral Rules and Regulations of the Mined Land <br /> Reclamation Board <br /> 6.4.3 Exhibit C- Pre-Mining and Mining Plan Maps of Affected Land <br /> 4. As required by Rule 6.2.1(2)(a) the Pre-Mining and Mining Plan Maps must show the name <br /> of the Applicant. Please update Exhibit C accordingly. <br /> 5. As required by Rule 6.2.1(2)(b) the Pre-Mining and Mining Plan Maps must be prepared and <br /> signed by a registered land surveyor, professional engineer, or other qualified person. <br /> Please sign the Exhibits C maps. <br /> 6. As required by Rule 6.2.1(2)(c) the Pre-Mining and Mining Plan Maps must give the date <br /> prepared. Please update Exhibit C accordingly. <br /> 7. As required by Rule 6.4.3(a), please identify the immediately adjacent surface owner of <br /> record northwest of the proposed mine site. <br /> 8. As required by Rule 6.4.3(g), please update Exhibit C to show the owner's name, type of <br /> structure and location of all significant, valuable, and permanent man-made structures <br /> contained on the area of affected land and within two hundred (200) feet of the affected <br /> land. <br /> Please include the following structure and easement owners, at minimum: <br /> a.
