Please add these features to this map and submit a <br />revised copy. <br />a. MCM Response: The referenced map has been revised and is included with the <br />responses. <br />b. DRMS Response: The North Spring on the revised Map #11 is in a different <br />place than depicted on the currently approved M #11. Please explain this <br />discrepancy. <br />Mid -Term #6; Adequacy Review Issues <br />3. Permit page 2.05 -7 contains a discussion of the drilling activities and planned future monitoring of <br />wells 94 -001 and 95 -14. Based on a review of current drill holes and hydrological monitoring sites, <br />the status of these wells is unclear. Hole 95 -14 is shown on Map 11 — Hydrologic Monitoring <br />Program Location Map but it is not sampled, included in the Annual Hydrology Report or shown on <br />AHR Figure 2 — Hydrologic Monitoring Program Location Map. The Division could find no other <br />reference to hole 94 -001. Please review the information on page 2.05 -7 for holes 94 -001 and 95 -14 <br />and update their status as necessary. Please provide the Division with a discussion of proposed <br />well 95 -14 and its possible inclusion in the hydrologic monitoring plan. <br />a. MCM Response: The specific Permit text discussion relating to the referenced wells has <br />been reviewed and revised to clarify and update the status and use of these wells. Copies <br />of the revised Permit discussion accompany these responses for replacement in the PAP. <br />b. DRMS Response: The information regarding this well was revised and placed on page <br />2.05 -2. The revised page indicated Well DPP -94 -001 was plugged and sealed and <br />released by CDRMS in 2012. A record of the abandonment of this well could not be <br />located. Please provide the abandonment report for this well. <br />c. MCM Response #2: Further review of the status of Well DPP -94 -001 indicates that it <br />was not abandoned and that it still exists. The relevant text has been revised to reflect <br />this correction and the revised page is included with these responses. MCM has <br />identified several old wells that remain from previous owners /operations, and is currently <br />developing plans to plug and seal these wells. <br />d. DRMS Response #2: The response indicates that this well still exists and that the <br />relevant text has been revised to reflect this correction. However, revised page 2.05 -2.1 <br />now includes the discussion of well DPP -94 -001 and it indicates this well has been <br />plugged and sealed as well as release from bond by the CDRMS. Please correct this <br />statement on the revised page to reflect the actual condition of the well. <br />
