The weather was partly cloudy and warm. The ground was damp
and the vegetation was still very green. It appears the mine site has been getting consistent precipitation. Stan
Muhr of Minrec, Inc was present for the inspection.
The purpose this inspection was to confirm the status of Abatement Step 2 of NOV- 2014 -001 (see enforcement
section below).
HYDROLOGIC BALANCE - Rule 4.05
Drainage Control 4.05.1, 4.05.2, 4.05.3; Siltation Structures 4.05.5, 4.05.6; Discharge Structures 4.05.7, 4.05.10;
Diversions 4.05.4; Effluent Limits 4.05.2; Ground Water Monitoring 4.05.13; Surface Water Monitoring 4.05.13;
Drainage — Acid and Toxic Materials 4.05.8; Impoundments 4.05.6, 4.05.9; Stream Buffer Zones 4.05.18:
The revisions approved in TR -28 have been properly constructed. The CMP that was buried below the P-
9 emergency spill way was removed. The P -9 primary/emergency spillway was re- directed to the refuse pile pond
ditch (photo 1). A ditch was constructed to direct water from Pond T -2 to the refuse pile pond as per the permit.
At the time of this inspection, water discharged from Pond T -2 was being directed to Pond P -9. P -9 contained
water but was not at the level to discharge to the refuse pile pond ditch yet. Water was not being directed to the
refuse pile pond ditch from Pond T -2.
The refuse pond was re- incorporated back into the permit area in TR -28 (see permit drawing D -3 -2). The pond
did not contain water (photo 2). The embankment was well vegetated, did not have any rills or gullies and
appeared to be stable (photo 3).
ROADS — Rule 4.03 Construction 4.03.1(3)/4.03.2(3); Drainage 4.03.1(4)/4.03.2(4); Surfacing and Maintenance
4.03.1(5) and (6)/4.03.2(5); and (6) Reclamation 4.03.1(7)/4.03.2(7):
Number of Partial Inspection this Fiscal Year: 1
Number of Complete Inspections this Fiscal Year: 0
Page 2 of 4
