PERMIT#: M-1996-049 <br /> INSPECTOR'S INITIALS: ECS <br /> INSPECTION DATE: July 17,2014 <br /> corrective action date of August 29, 2014 an RTB letter will be issued to the permittee. <br /> CORRECTIVE ACTION DUE DATE: 8/29/14 <br /> INSPECTION TOPIC: Signs & Markers <br /> PROBLEM/POSSIBLE VIOLATION: Problem:The affected area boundary markers are missing or incorrectly <br /> placed. This is a problem for failure to maintain boundary markers around the affected area as required by <br /> Section 3.1.12(2) of the rule. <br /> CORRECTIVE ACTIONS:The operator shall conduct a survey and replace the boundary markers in the correct <br /> location(s). The operator shall provide proof to the Division that this has been done by the corrective action <br /> date. <br /> CORRECTIVE ACTION DUE DATE: 8/29/14 <br /> OBSERVATIONS <br /> This was a routine monitoring inspection of the Maryland Creek Ranch 112c permit conducted by Eric Scott of <br /> DRMS. DRMS was accompanied on the inspection by Johanna Hopkins and John O'Hara of Everist Materials. <br /> This site is a 216 acre active 112c permit located approximately 4 miles north of Silverthorne on the west side <br /> of Hwy 9. This site is divided into 5 primary areas. These are described below starting at the north end of the <br /> permit and moving south. <br /> The north end of the site is largely reclaimed to rangeland with several small unlined lakes. There is also large <br /> sediment settling pond currently in use in this area. <br /> South of the settling pond is the "process area" of the site. This area is surrounded by large vegetated earth <br /> berms on the south and east sides as required by the county. The site offices, washing and sorting plants, <br /> concrete plant, asphalt recycling plant, and shop are all located in this "process area". It was noted that one <br /> of the stockpiles located on the west border of the permit appears to have partly slid out of the permit area. <br /> This has been noted as a possible problem/violation on the first page of this report and will need to be fully <br /> addressed by the corrective action date shown. The bond amount for this process area will also need to be <br /> revised to include the removal of all structures, unless the operator wishes to change the post-mining land use <br /> to "industrial/commercial" through the amendment process. <br /> South of the process area is an un-mined area in the middle of the permit that is still being utilized as a <br /> ranch/grazing area. This are will likely be mined in the future as the active mining area moves north. <br /> South of the un-mined ranch area is the active mining are of the site. Material is being mined from this area <br /> using a dredge and then conveyed to the process area by belt for washing and sorting. The approx. 9 acres <br /> added to the affected area in the last amendment is located in the southwest corner of this active area and <br /> has not yet been disturbed. <br /> At the south end of the site is the previously mined area consisting of an un-lined open lake. This lake is <br /> currently being backfilled from the south using inert fill (primarily old concrete). As the backfilled area <br /> expands and is leveled it is then reseeded to rangeland. <br /> No issues other than the ones identified on the first pages of this report were noted. The corrective action <br /> date set at this time for the resolution of all the possible problems/violations noted is August 29, 2014. <br /> Page 2 of 4 <br />
