The Applicant submitted one map for Exhibit E; Rule 6.3.5(1) states that a map must be <br /> submitted for the Mine Plan and the Reclamation Plan. A Mining Plan Map must be <br /> submitted and shall include the following elements: <br /> a) A map title(Mining Plan Map) <br /> b) The name of the Applicant <br /> c) The map must be signed by the person who prepared it <br /> d) Give the date prepared <br /> e) Label the permit boundary(corresponds to target areas) <br /> f) The map must include a scale. Alternately,the Applicant can specify the length of each <br /> side of the permit boundary. <br /> g) Label the names of the owner(s) of record of the surface of the affected area and of the <br /> land within two hundred feet of the affected area. If all surrounding land is owned by the <br /> US Forest Service, then you may make a note of such on the map. <br /> h) Outline and label overburden stockpiles. <br /> 14. The Reclamation Plan Map must be revised to include the following elements: <br /> a) A map title(Reclamation Plan Map) <br /> b) The name of the Applicant <br /> c) The map must be signed by the person who prepared it <br /> d) Give the date prepared <br /> e) Label the permit boundary <br /> f) The map must include a scale. Alternately, the Applicant can specify the length of each <br /> side of the permit boundary. <br /> g) State the average thickness of replaced overburden; you may make a note on the map that <br /> states how much overburden will be replaced, i.e. between 6 to 8 feet of overburden will <br /> be replaced. <br /> 6.3.6 Exhibit F—List of Other Permits and Licenses Required <br /> 15. The Applicant has stated a Plan of Operations (POO) will be obtained from the US Forest <br /> Service. Please commit to providing the Division with a copy of the approved POO. <br /> 6.3.7 Exhibit G—Source of Legal Right to Enter <br /> 16. As required by Rule 6.3.7, please provide a description of the basis for legal right of entry to <br /> the site and to conduct mining and reclamation for Owners of Record. This may be a copy of <br /> an access lease, deed, abstract of title, current tax receipt or a copy of a BLM lode claim. <br />
