Please update <br /> these coordinates to reflect the actual entrance to the mine. Under the requirements of Rule <br /> 6.4.1,coordinates of the primary mine entrance location should also be included in Exhibit A. <br /> - The coordinates of the mine entrance have been modified.A new page 2 is attached <br /> with the revised numbers.An aerial photo depicts where those coordinates place the <br /> entrance. <br /> 2) The certification page was not completed. Philips County informed the Division of this and <br /> emailed a scanned copy of the signed certification page. After review of the scanned page, <br /> it appears that the notary's stamp was not included. The Division has also not received the <br /> original signed certification page in the mail. <br /> - Attached are two signed certification pages. <br />
