1.0 Purpose: <br />A plan to implement erosion control and maintenance on an historic waste dump site <br />during site exploration and reclamation operations and to satisfy the requirements of the <br />Colorado Department of Human Health and Environment, Water Quality Control <br />Division, Stormwater Permit requirements. <br />2.0 Responsibility: <br />New West Reclamation, LLC. <br />P.O. Box 1798 <br />Leadville, CO 80461 <br />Phone: 303 - 681— 9344 <br />FAX: 303 - 681 — 2117 <br />3.0 Procedures: <br />Stormwater management for this Project shall be performed in compliance with the <br />Colorado Department of Public Health and Environment (CDPHE), Colorado Discharge <br />Permitting System - Stormwater Construction Permit (CDPS -SCP) and the Stormwater <br />Management Plan (SWMP). <br />Installation of Best Management Practices (BMP) shall be completed by New West <br />Minerals (Operator). The Operator shall install the BMP's as shown on Figures #1, 2 and <br />3. These figures should be considered as a generic example of our proposed BMP's. <br />Once operations begin at a site, the Operator will stockpile or otherwise have available <br />BMP materials for installation upon completion of a site sample sequence. A site <br />sampling sequence will consist of the following steps: <br />1) Identify a sample site on the historic waste dump. <br />2) Determine the method of excavation necessary to obtain ore samples from the <br />sample location. <br />3) Install the tarp or other suitable material for placement of excavated material. <br />(Surface barrier material would not be used for hand or auger samples, but would <br />be used for sampling using a backhoe.) <br />4) Excavate material from sample site location. <br />5) Obtain sample(s). <br />6) Replace excavated material and tamp/blend the surface into the adjacent waste <br />dump surface. <br />7) Place and secure burlap or other suitable material on the surface of the waste <br />dump sample site location.
